Intake Registered Nurse (RN) Managed Long Term Care (MLTC)
Compensation: $95K$105K per year $20K SOB
Job Type: Full-time Monday through Friday schedule
Location: Bronx NY
Our client is seeking an experienced and compassionate Intake Registered Nurse (RN) to join their Managed Long Term Care (MLTC) team. This role is ideal for a clinician passionate about supporting older adults and ensuring they receive the care and services needed to live safely and independently at home. The Intake RN plays a key role in evaluating potential members determining eligibility and facilitating a smooth transition into managed long-term care programs.
Key Responsibilities
- Conduct community-based outreach and assessments to determine potential members eligibility and safety for program enrollment.
- Collaborate with intake outreach and clinical teams to coordinate appropriate care and services for new members.
- Educate potential members and their families about program benefits enrollment processes and available services.
- Maintain timely and accurate documentation of all assessments and communications in accordance with program standards.
- Serve as a liaison between members families physicians and referral sources to ensure a seamless intake experience.
Qualifications
- Active New York State Registered Nurse (RN) license.
- Associates Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
- Minimum of 1 year of nursing experience in an acute care setting.
- UAS-NY (Uniform Assessment System New York) experience required.
- Home care or care management experience preferred.
- Proficient in Microsoft Office Suite (Word Excel Outlook).
- Excellent verbal written and interpersonal communication skills.
- Bilingual in Chinese or Spanish required.
