Job Summary:
The Nurse Practitioner (NP) provides advanced clinical assessment triage and care management in a clinic setting. This role includes evaluating patient concerns prioritizing care based on acuity initiating treatment plans and collaborating with healthcare providers to ensure timely effective patient care. The NP plays a key role in clinical decision-making patient education and promoting continuity of care.
Hours:
- .6 FTE - 24 hours per week
- Monday - Thursday - 17:00 - 23:00
Essential Job Functions:
- Advocate for patients rights preferences and needs ensuring that they receive high-quality compassionate care that is tailored to their individual circumstances
- Project a professional demeanor and appearance while maintaining the confidentiality of the patient coworkers and the hospital
- Perform clinical assessment triage and management of acute and chronic conditions in accordance with established protocols
- Prioritize and respond to patient concerns via phone electronic messages or in-person visits
- Develop and implement evidence-based treatment plans including ordering diagnostics prescribing medications and arranging follow-up care
- Collaborate with providers registered nurses and support staff to coordinate care and ensure timely patient interventions
- Document clinical findings assessments and care plans accurately in the electronic health record
- Provide patient education on diagnosis medications and self-care strategies
- Participate in patient care huddles and interdisciplinary team meetings to optimize patient care
- Participate in quality improvement initiatives protocol development and clinical training as needed
- Follow infection control and safety protocols maintaining a high standard of care and professionalism
- Utilize the organizations secure messaging platform in accordance with departmental protocols including but not limited to receipt and timely responses to communications related to on-call responsibilities etc.
- Perform other duties as assigned
Knowledge Skills & Abilities:
- Medical Terminology
- Electronic Medical Records (EMR) systems
- Quality assurance and accreditation standards
- Understanding of HIPAA regulations patient confidentiality laws and patient rights
- Effective English communication in verbal and written formats
- Competent in conscious sedation according to hospital policy
- Quality work including attention to detail and accuracy
- Basic computer proficiency
- Carry out hospital customer service standards
- Prioritize tasks effectively through managing workload based on urgency and importance
- Capacity to work independently and as part of a multidisciplinary team
- Continuously learn and improve through staying up to date on job-specific trends policies and new technologies
- Successfully demonstrate organization-wide performance review competencies
Qualifications:
- Demonstrates eligibility for employment in the U.S.
- Masters or Doctoral degree in Nursing (MSN or DNP) from an accredited program required
- Current Nurse Practitioner (NP) licensure in the state of Wisconsin required
- Current Basic Life Support (BLS) certification required
- Must obtain ACLS and PALS certification within six months of hire
- 3840 hours of clinical hours of advanced practice registered nursing practice in recognized role while working with a physician during those 3840 hours of practice required
- Two (2) years of clinical experience preferably in triage primary care urgent care or a similar setting required
- National certification as an NP (e.g. AANP ANCC) in relevant specialty preferred
- Must possess a cell phone that interfaces with the organizations secure messaging system
