Position Summary

In accordance with established standards professional requirements and as a member of the multidisciplinary health care team the Discharge & Flow Coordinator provides administrative and clinical leadership to all staff in a supportive empowering manner while facilitating the effective and efficient movement of admitted patients through the hospital system. Responsible for planning organizing directing coordinating and evaluating activities related to bed utilization and patient discharge. The primary site will be based out of Toronto Western Hospital. UHN has multiple locations across Toronto and you may be required to work at other locations periodically as needed. 

Duties

  • Participating in implementing and coordinating a patient-centered care plan with patients family members significant others and the multi-disciplinary team for the purpose of facilitating the patients movement through the continuum of care.
  • Providing a link between the hospital community agencies and alternate level of care facilities
  • Reviewing incoming applications monitoring chronic care nursing home application
  • Liaising and establishing contact with members of the multi-disciplinary team and appropriate community agencies to obtain provide or exchange information relating to patients psycho-social functioning and health care in the past present and future in conjunction with the clinical team
  • Educating hospital staff to the various aspects of discharge planning and available resources; acting as a clinical resource for the acute inter-professional teams by providing information based on clinical knowledge and theory/practice related to clients with complex health care needs
  • Coordinating the daily bed requirements of patients being admitted/transferred to the hospital
  • Establishing and maintaining a systematic review and assessment of bed availability
  • Ensuring appropriate patient assignment based on clinical need working with other departments to assess the need for standards to optimize the utilization of beds (inclusive of pre-operative days and direct admissions).

Qualifications :

  • Completion of Bachelors in Health related discipline or recognized equivalent
  • Current valid Certificate of Practice from relevant Health related College
  • Five (5) years practical and related clinical experience
  • Knowledge and experience in discharge planning and case management
  • Knowledge and experience using utilization management principles and length of stay management strategies
  • Experience in leadership and project management preferred
  • Experience in program development and implementation preferred
  • Excellent organizational interpersonal and communication skills
  • Excellent analytical skills decision making problem recognition and solving skills
  • Excellent time management skills with ability to multi-task
