Job Summary

We have an exciting role on offer within ourCommissioning Service as a Trusted Assessor.

About Us

The Commissioning Team is based within Adult Social Care but works in partnership with service directorates across the Council to support key service objectives.

The Trusted Assessment team play an integral role in preventing unnecessary admission into Hospital as well as assisting in an appropriate timely discharge onto a care setting that meets the persons needs.

The Trusted Assessor role may be right for you. The post will undertake assessments on behalf of Residential and Nursing Care Homes in order to facilitate safe and timely discharges from Hospital and for those requiring a Care Home who already live in the community. This maybe for short term care rehabilitation or long term care.

The Council has a policy that supports new ways of working this post will embrace a mix of home working and outstation working from places such as Hospitals core offices.

We need you if you have experience in working in the Health and Care sector have excellent interpersonal skills and experience in multi-agency working. You will work in a fast-paced team to deliver the best results.

This isa 37 hours a week role and will be delivered over both weekdays and weekends. This will include enhancements for weekend work.

A full UK Driving License is essential for this role with proof of Business Insurance is required prior to starting if not already in place.
