General Summary:
The Case Manager is responsible for providing case management services including providing information referral and support to clients of Outpatient Mental Health Center (OMHC) who have identified needs related to social determinates of health.
Principal Responsibilities and Duties:
- Coordinate referrals to services including medical care housing entitlements food clothing and safety needs per established policies.
- Complete all documentation as required within established time frames.
- Will respond to phone calls and crisis walk-ins of potential participants.
- Screen and assess potential participants using Social Determinates of Health Questionnaire.
- Provide case management to link participants with community resources.
- Assist clients with completing applications as needed.
- Manage required data collection.
- Make referrals to program supportive services and job/career enhancement services.
- Inform treatment team of progress on case management activities.
- Coordinate with other providers including health home nurses & nurse care coordinators.
- Prepare reports.
- Develop database of case management resources.
- Attend regular staff meetings and supervision as scheduled.
- Travel as needed to various program sites or to sites of community stakeholders.
- Performs all other direct job-related duties as is assigned.
The pay range for this position is $20.00 minimum to $23.00 maximum. Pay for this position is determined on a number of factors including but not limited to years and level of related experience.
Knowledge Skills and Abilities Required:
- BA/BS in social work counseling psychology or other mental health or social service related field.
- 2 years related experience required in lieu of BA/BS degree. Community health worker experience a plus.
- Knowledge and understanding of mental health conditions and person-centered approach.
- Good organizational skills and ability to multi-task.
- Good verbal and written communication skills.
- Basic computer skills to include knowledge of Word & Excel.
- Valid Maryland drivers license. Must have a driving record with 3 or less points on it at the time of hire.
