Responsibilities:
- Oversee and coordinate tailored care plans ensuring that our clients receive appropriate and timely services.
- Serve as a strong advocate for clients ensuring their needs are met in home medical and care facility settings including while communicating with healthcare providers.
- Work closely with team members to address clients needs and contribute your expertise to the team helping to enhance the collective knowledge and skills.
- Participate in after-hours or weekend on-call rotations as needed to provide continuous support to clients.
- Update family members and other members of the care team and maintain thorough and accurate documentation. Approximately 90% of the role involves field work with 10% dedicated to follow-up and office-based tasks.
Requirements:
- A masters degree in a related field or a bachelors degree with at least 2 years of relevant experience in gerontology mental health home care or similar.
- Available 9am to 6pm Monday-Friday and some weekends.
- Dependable adaptable and accountable with a strong commitment to providing exceptional care and support to clients.
- Must be able to lift and maneuver up to 50 pounds.
- Must be comfortable and willing to drive regularly around the Virginia and DC Metro area.
