Job Duties
Oversee the admissions and discharges flow of the hospital maintaining awareness of bed status and barriers to discharge.
Serve as an active member of the interdisciplinary team providing a social work perspective.
Plan organize and implement a full range of clinical services to patients and their families including initial and ongoing assessment individual family and group counseling.
Educational needs assessment and provision of education as indicated.
Coordination of discharge planning with the Community Service Board (CSB).
Minimum Qualifications
Experience as a social worker in an inpatient healthcare setting working directly with patients.
Masters degree in social work from an accredited college/university required.
Knowledge of admission and discharge processes.
This position will also include:
Knowledge of population specific factors in assessment and treatment.
Knowledge of theories of human behavior and development.
Skills and ability in providing individual family and group counseling.
Knowledge of community resources.
Advance interviewing skills.
Knowledge of Social Work Code of Ethics.
Ability to make independent work decisions.
Ability to independently maintain clinical records on assigned cases in accordance with hospital policies and governing body regulations.
Knowledge of SVMHI admissions process.
Successfully complete and maintain throughout employment CPR Therapeutic Options (TO) training and other required training as required by the facility.
Additional Considerations
Veterans and people with National Service.
Bilingual fluency in Spanish and English.
