In this role the Patient Navigation Coordinator within our Care Coordination department will support clinicians within the section of Hospital Medicine as they provide services to patients hospitalized at the medical center. As a member of the multidisciplinary team this position will support the clinical staff to promote timely patient care enhance the communication with families other providers and external organizations. The Patient Navigation Coordinator will improve the transition of care process and enhance compliance with payer related tasks. This position will support all hospitalist providers on day services patients and families case management and social work assigned to the hospitalist team. This position will also identify areas for improved efficiency of administrative processes.

Patient Navigation Coordinator Education and ExperienceRequirements:

  • A Bachelors degree from an accredited academic body is Preferred.
  • Experience working in a healthcare setting and supporting the patient experience is highly preferred.
  • Must be proficient using the latest versions of Microsoft Office Suite and web searches.

Compensation & Benefits Overview

UChicago Medicine is committed to transparency in compensation and benefits. The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
