POSITION SUMMARY

Working as part of a multidisciplinary team and under the general direction of the Manager Home and Community Care the Patient Service Clerk (PSC) for Home and Community Care is responsible for assisting the Care Coordinators with operational functions providing clerical support. The Patient Service Clerk for Home and Community Care provides information and referral and demonstrates a high level of client service to the Home and Community Care team other CHEO staff and physicians families service providers and the general public ensuring that they feel heard understood and are confident of follow-up. The Patient Service Clerk needs to be available to work any shifts including evenings (until 20:00hrs) holidays and weekends.

MAIN RESPONSIBILITIES

  • Receives and initiates incoming and outgoing telephone calls.

  • Communicates with members of the team to ensure information is distributed and obtained appropriately including collecting sorting and distributing mail courier packages forms lists and documents to and from staff and agencies and photocopying and faxing/scanning documents and forms as .

  • Maintains an efficient current and secure CHRIS client record in accordance with established guidelines including completing entry of all required information (ie scanning and data entry) documenting all actions taken in the client file ensuring file availability when required by Care Coordinators assigning new and transferred client files to the appropriate Care Coordinator uploading documents to CHRIS library accessing the client information system for information as required processing files for all discharges.

  • Listens to and documents in client file Care Coordinators voice mail and in accordance with established guidelines takes and relays messages refers calls and arranges for decreases in services and collection of equipment.

  • Processes requisitions prepares related documents and form letters monitors inventory of and orders supplies in accordance with established guidelines.

  • Updates client files by entering information which corresponds to standardized pathways or as per Care Coordinator direction.

  • Liaises with staff care providers families and pharmacies regarding confirmation of dates deliveries discrepancies and delays of supplies increases decreases and discharges in service and takes appropriate clerical action in accordance with established guidelines.

  • Books routine patient home visit appointments and patient/family conferences prepares agenda and reserves meeting rooms/books online meetings as directed by Care Coordinators.

  • Arranges patient transportation by taxi or ambulance in accordance with established guidelines and as authorized by Care Coordinators.

  • Assists with projects or initiatives related to job responsibilities participates in orientation and committees and performs other related duties as requested by their Manager.

  • Acts as a resource for CHEO employees service providers school personnel and community.

  • Attends and participates actively in team meetings and general staff meetings.

  • Assumes responsibility for maintaining professional knowledge and updated skills.

  • Perform work in accordance with the provisions of the Occupational Health and Safety Act and Regulations and all CHEO corporate/departmental policies and procedures related to Occupational Health and Safety.

  • Perform other related duties as assigned by supervisor.

QUALIFICATIONS

  • Secondary School Graduation Diploma (Essential)

  • One (1) year Community College diploma in Office Administration Business Health Administration or equivalent program (Preferred)

  • A minimum of six (6) months of related professional experience in a clinical administrative setting (Essential)

  • Two (2) years of related professional experience (Preferred)

  • Demonstrated knowledge of office procedures including filing and record maintenance and operation of telephone systems and office equipment (Essential)

  • Strong skills related to word processing and advanced proficiency with Microsoft Office Suite (including Excel PowerPoint Word) (Essential)

  • Awareness of and sensitivity to the needs of a culturally diverse population (Essential)

  • Flexible and adaptable with effective problem-solving skills (Essential)

  • Communicates effectively orally and in writing in both official languages (Essential)

  • Demonstrates excellenct interpersonal skills (Essential)

  • Excellent time management and organizational skills with the demonstrated ability to perform efficiently with attention to detail (Essential)

  • Works effectively in both a team environment and independently as (Essential)

  • Diplomacy in dealing with sensitive and/or confidential issues (Essential)

Effective March 29 2021 CHEO will conduct a more comprehensive pre-screening and evaluation process. This process may include more varied and multidimensional testing such as a written test practical exam or other assessment. All candidates who meet the essential and preferred qualifications may be tested. Bilingualism (French/English) will be validated by a language proficiency assessment conducted by a third party provider. Candidates will be required to meet the minimum linguistic profile prior to the date of hire.

Faisant partie dune équipe multidisciplinaire et sous la direction générale du gestionnaire Prestation soins à domicile et en milieu Communautaire le commis des services aux patients est responsable daider les coordonnateurs des soins à accomplir les fonctions opérationnelles en fournissant un service de soutien de bureau. Le commis des services aux patients de la Prestation soins à domicile et en milieu Communautaire fournit de linformation et des aiguillages et offre un service à la clientèle de niveau élevé au sein de léquipe de Prestation soins à domicile et en milieu Communautaire dautres employés et des médecins de CHEO ainsi que les familles les fournisseurs de service et le public en général en veillant à ce quils sentent quils sont entendus et compris et quils aient confiance à légard du suivi.Le commis des services aux patients doit être disponible pour travailler tous les quarts de travail y compris les soirs (jusquà 20 h) les jours fériés et les fins de semaine.
