Hearts at Home is a new virtual program supporting early discharge and reducing avoidable hospital admissions for people living with heart program will be supporting our private patients from Northside Hospital Brisbane but as a virtual program and can be delivered from any state.
About the Role
We are looking for an enthusiastic Physiotherapist with demonstrated cardiac/cardiopulmonary experience to join our team.
Hearts at Home Physiotherapist (Grade 2)
Fixed Term: 12 moths engagement
Hours: 0.2 FTE (Approximately 7.6 hour a week)
Location: Melbourne OR Brisbane
Key Responsibilities:
Deliver high-quality evidence-based physiotherapy assessment and exercise prescription for heart failure patients.
Conduct virtual (phone/video) assessments and monitor health outcomes.
Use technology to support patient care and goal setting.
Collaborate with hospital primary care and multidisciplinary teams.
Maintain accurate documentation and contribute to service development.
About You:
Minimum 5 years post-registration experience including 3 years of cardiac/cardiopulmonary experience.
Strong clinical assessment and digital care delivery skills.
Excellent communication and teamwork.
Bachelor or Doctor of Physiotherapy (or equivalent) and current AHPRA registration.
