The Support Worker will provide individualised person-centred support in a one-to-one capacity with a focus on promoting independence wellbeing and quality of life. This role requires a strong mental health background the ability to work independently and flexibility to meet rostered hours including sleepover shifts.
Deliver personalised daily living and community support in line with participants support plan.
Apply mental health knowledge to provide safe empathetic and effective support.
Work independently and exercise sound judgment while supporting participants in a 1:1 environment.
Assist with routine activities community participation and skill development.
Support participants during sleepover shifts ensuring safety and wellbeing overnight.
Maintain accurate case notes shift reports and documentation in line with organisational policies.
Adhere to all safeguarding workplace health and safety and quality compliance requirements.
Communicate effectively with the team family members and external stakeholders where appropriate.
Demonstrated experience as a Support Worker within the disability mental health or community services sector.
Strong understanding of mental health needs and ability to implement appropriate support strategies.
Commitment and ability to work the required rostered hours as per the template (non-negotiable).
Ability to work independently with minimal supervision in a 1:1 support setting.
Experience in delivering sleepover/overnight shifts.
Current First Aid and CPR certification.
NDIS Worker Screening Check Working with Children Check and National Police Check (or willingness to obtain).
Current drivers licence and reliable vehicle.
Certificate III or IV in Individual Support Mental Health Disability or equivalent qualification.
Experience supporting individuals with complex needs.
Full-Time
