Job Summary

Eden Health is hiring a Patient Experience Coordinator responsible for enhancing and supporting the culture of service excellence within Eden Health through interaction with patients providers and staff in a variety of methods which will drive improvement in patient staff and provider experience. The Patient Experience Coordinator will exemplify the tenets of the Studer Principles. The Patient Experience Coordinator is also responsible for ensuring the efficient and effective resolution of complaints and grievances.

Job Summary:
  • Collaborates with leadership to determine measurement methods analyze processes and manage program related to patient staff and provider satisfaction.
  • Acquires analyzes and reports information specific to the patient experience through various feedback channels with the goal of better understanding and improving the organizations customer experience performance.
  • Oversees patient staff and provider survey data as well as additional information such as external website feedback customer comments and similar types of information.
  • Accurately and appropriately documents communications with patients staff and providers.
  • Assists in evaluating performance; monitors success of patient staff and provider experience initiatives.
  • Provides agency staff and leadership with useable and understandable resources and/or training related to improving the patient experience.
  • Collaborates with agency staff/leadership to assist in complaint/grievance resolution; tracking trending and managing complaints and grievances; and performs investigation and response documented within the Eden Health event reporting system as directed.
  • Provides on-the-spot problem solving as part of communications with patients and documents as appropriate.
  • Provides summary reports of program activities and outcomes as specified.
  • Works with vendor account managers and representatives to optimize feedback methods and patient experience improvement activities and meet government mandated reporting initiatives.
  • Contributes to improvement projects related to patient satisfaction scores and star ratings where applicable.
  • Identifies potential exposure for risk and assists with implementation of systems to avoid exposure.
  • Attends Agency QAPI meetings routinely offering suggestions for appropriate projects and processes related to the patient experience.
Education/Experience Requirements:
  • High school diploma or equivalent.
  • Health care worker with a current license from the corresponding State Board (preferred).
  • Experience in community health home health and/or hospice services (preferred).
  • Experience with surveys and survey methodology (preferred).
  • Prior experience in program development (preferred).
