**Qualifications Required**
- Board Certification in Obstetrics and Gynecology (Board Eligible Status Not Accepted)
- Required
- Michigan Medical License and Controlled Substance Registration (CSR) at Time of Submission
- Required
- Basic Life Support (BLS) Certification
- Required
- Providers must be local to the area
- Required with no exceptions
- A clean background is mandatory for expedited credentialing
- Required with no exceptions
