Summary of Duties
Aging and People with Disabilities:
Our vision is that Oregons older adults people with disabilities and their families experience person-centered services supports and early interventions that are innovative and help maintain independence promote safety and well-being honor choice respect cultural preferences and uphold dignity. The services provided help people access the supports they need to live their best lives in the communities they choose and at every stage of life.
As a Case Aide you will:
Assist Case Managers with Long-Term Care Programs:
This includes requesting/triaging collateral information both in person and/or on the phone.
Assess the physical and cognitive needs of Oregonians applying for or receiving long-term care services
Recognize and evaluate health and functional status including identifying signs of potential abuse or neglect
Support the development coordination and arrangement of services that ensure continuity of care for older adults and individuals with disabilities
Help families in Oregons assistance programs achieve self-sufficiency while meeting their basic needs.
Utilize all applicable forms computer systems tools and resources and upon report any changes to the direct case manager.
Create packets that need to be mailed out for all circumstances needed for assessments.
Make appropriate referrals to other ODHS programs and resources as well as community resources.
Consult immediately with case manager and or supervisor when identifying problematic or unsafe in-home plans or significant changes in clients health and safety needs.
Assist in ensuring all service deliveries are provided in a culturally competent and trauma-informed way; provide communication to Oregonians in their preferred language/format by utilizing printed materials available in different languages/formats and use of language services available.
Consult immediately with case manager and or supervisor when identifying problematic or unsafe in-home plans or significant changes in clients health and safety needs.
Minimum Qualifications
One year of experience assisting professionals in one or more of the following specialized social program areas:
Economically disadvantaged
Substance use and dependency
Disability Services
Child abuse and neglect
Juvenile delinquency
Any degree in a related mental health or human services program may substitute for six months of the required experience.
At the time of a recruitment the recruiting agency will specify the required program area(s).
Essential Attributes
We are looking for candidates with:
Experience collaborating effectively with Case Managers clients and service providers.
Experience assessing client needs and navigating sensitive situations.
Experience applying sound judgment and discretion while applying policy and rules when documenting concerns and initiating appropriate interventions.
Experience conducting observational assessments to identify physical emotional or environmental indicators of mistreatment or unsafe conditions.
Experience utilizing appropriate forms computer systems and tools to support case management.
Experience efficiently managing a workload in a constantly changing work situation as well as plan for and resolve complex problems.

Working Conditions
Work Locations: Aging and People with Disabilities (APD) Office in Oregon City long-term care facilities and throughout the broader community.
Hours: Monday Friday 8:00 AM to 5:00 PM; occasional overtime may be needed including evenings and weekends.
Travel: Occasional travel for meetings trainings and other case related activities. This includes required occasional overnight travel.
Physical Requirements: Repetitive motions prolonged sitting daily use of computers and technology and occasional lifting.
This role involves handling sensitive topics related to trauma abuse and crises requiring a trauma-informed approach that is essential to ensure a safe environment.
You may interact with individuals who have experienced trauma and may have difficulty managing their emotions.
Be prepared for stressful situations that require quick decision-making to ensure safety for yourself and others.

The Oregon Department of Human Services does not offer visa sponsorship. Within three days of hire you will be required to complete the US Department of Homeland Securitys I-9 form confirming authorization to work in the United States. Must possess a valid drivers license and acceptable driving record to use state vehicles. Reasonable accommodations per the ADA or creative solutions in certain cases may be used to meet this need.
