Join us as a Service Coordinator
As a Service Coordinator youll be responsible for the overall coordination and provision of DABS Inc services to its consumers. This includes managing the day-to-day operations of staff who work directly with clients in their homes or in the community. Key duties involve staff supervision scheduling monitoring client progress reviewing documentation and collaborating with families conservators and Regional Centers to ensure all individualized consumer needs are met.

What Youll Do

    • Staff Supervision
    • Lead and supervise direct care staff including ongoing training performance management disciplinary actions and scheduling.
    • Ensure adequate staff coverage at all times according to individualized service plans and budgets personally working shifts when a replacement cannot be found.
    • Provide continuous guidance on DABS policy and procedures as well as contractual and regulatory obligations.
    • Case Management
    • Manage a caseload of at least 8 consumers serving as their primary advocate and ensuring effective implementation of their Individual Program Plans (IPP).
    • Develop and write all Individual Support Plans (ISP) for the respective caseload and submit required updates (annual semi-annual and quarterly) to the Regional Center.
    • Actively participate in internal consumer-related and other mandated meetings.
    • Perform regular check-ins confirming consumer satisfaction and receipt of all provisions agreed upon in their IPP.
    • Maintain current and accurate services plans (ISP behavior and/or health plans) and corresponding consumer data.
    • Coordinate and maximize social assistance benefits for consumers including IHSS and Section 8 ensuring applications are completed and benefits are maintained.
    • Provide immediate on-site response in the event of consumer or staff emergencies.
    • Administration and Compliance
    • Create and maintain electronic staff schedules in compliance with Electronic Visit Verification (EVV) mandates.
    • Regularly review timesheets ensuring timely and accurate submission.
    • Prepare detailed and accurate documentation including but not limited to shift notes and Special Incident Reports (SIRs).
    • Adhere to all company policies and relevant regulations including Title 17 & 22 and act as a Mandated Reporter.
    • Assume any other administrative responsibilities assigned.

What Youll Have

    • 3 years of experience in supporting adults with developmental disabilities with at least 1 year of experience in service coordination and/or staff supervision.
    • Excellent verbal and written communication skills with the ability to effectively interact with a diverse range of individuals including staff superiors consumers and external partners.
    • A high degree of discretion when handling sensitive and confidential information.
    • Strong initiative and independent judgment to organize prioritize and manage multiple projects and deadlines with minimal supervision.
    • Proficiency in common office software including Microsoft Word Excel and PowerPoint as well as cloud-based applications.

Requirements

    • Able to work early mornings evenings and weekend days as needed.
    • Able to drive participants in ones own personal vehicle; possesses a valid drivers license and auto insurance.
    • Able to travel to all cities in San Mateo County.
    • Able to pass a background check with satisfactory results.
    • Able to provide current CPR/First Aid certification and TB clearance.
    • Able to assist individuals with physical needs including lifting pushing and pulling up to 60 lbs.
    • Able to respond to and assist individuals during medical events such as seizures.
