Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute sub-acute aged care and community services.

NHW have an exciting opportunity for a passionate and experienced staff member to join our team as a Sub-acute Ambulatory Care (SACS) Co-ordinator.

In this vital role you will provide day-to-day operational leadership and coordination across the SACS and Transition Care Program (TCP) services. The role is key to delivering high-quality goal-directed care that supports clients independence promotes hospital admission avoidance and ensures smooth patient flow throughout Northeast Health Wangaratta (NHW).

As SACS Co-ordinator you will:

Support and guide staff in delivering holistic person-centred care.
Work collaboratively across NHW departments and with external services.
Promote NHW as a leading regional health care provider.
Lead with compassion initiative and a strong team-based approach.

Wangaratta has a population of approximately 30000 and our catchment serves a community of around 100 000 throughout the northeast.
