  • Salary: 28598 per annum (pro rata for part time)

  • Hours: 37 hours per week - flexible working options available

  • Contract:Permanent

As a Family Services Assistant Coordinatorin theSEND (Special Educational Needs and Disabilities)EHCP (Education Health and Care Plan) Review Hubyoull be as keen to collaborate and build a bright future as we the possibilities.

Your roleand responsibilities:

  • work efficiently and effectively with the CYP (Children and Young People Service) supporting children and young people with SEND and their families along with partner agencies to provide support through the SEND journey as well as ensuring statutory timescales are met

  • provide a person-centred approach to ensure that ECHPs are supportive to the child or young persons needs captures their views and provides agreed outcomes to ensure children and young people with SEND are making progress

  • triage Annual Review documents that come from settings and assign to others using our agreed criteria and processes making sure to write detailed and informative case notes for colleagues.

You should apply if you are skilled in working with people and can work flexibly to meet the demands of a challenging yet rewarding role.

You will need:

  • suitable qualifications at Level 3 and above or equivalent experience in a relevant area of work

  • evidence of CPD (continuing professional development)

  • experience of collaborative work with CYP families and professionals and evidence of using solution focused approaches in practice

  • a key understanding of Child Protection and Safeguarding procedures.

The team

The SEND Family Services Team leads on all aspects of our family facing work and supports individual children and young people with SEND and their families throughout their SEND journey. This includes securing the right education offer to support their specific needs and ensuring regular and effective review of their progress holding providers to account across the system for meeting the needs of individual children and young people.

The team includes practitioners from a range of backgrounds who provide a wealth of knowledge and experience and support partnerships across the education health and care system.
