Job Summary

  • Highschool diploma or equivalent and 2 years in a transfer center or patient access department required or
  • Graduate of an accredited LVN program required or
  • Graduate of an accreted paramedic program required
  • No experience required if LVN or Paramedic
  • 2 years related experience in critical care emergency services or patient access services preferred
  • LVN (if LVN) upon hire required

What You Will Do

The Patient Logistic Centers Specialist is responsible for the coordination of resources needed to transfer a patient throughout the THR system. This role involves collaboration with various departments including internal and external departments such as providers charge nurses admissions and clinical teams and central bed placement to facilitate timely and appropriate patient placements. The Patient Logistic Centers Specialist is responsible for coordinating and managing logistic center needs across various departments across the enterprise.

Bed Management: Assess incoming patient information to determine appropriate bed placement based on medical needs bed availability and hospital resources. Monitor bed board. Collaborates with entity administrative supervisor Patient Logistics Patient Access Nursing staff and physicians to ensure proper patient placement. Facilitate doc to doc.

Assumes responsibility for professional growth. Obtains required annual continuing education. If applicable keeps professional license up to date. Stays up to date with departmental changes by attending monthly staff meetings and reading emails. Completes learning lessons Performance Evaluation (self eval and meets with leader) or licensure requirements (if applicable) before their due date. With approval by leadership contributes to departmental or organizational projects. Complete assigned monthly audits.

Utilize department digital platforms effectively: Telephone systems tracking systems electronic communication systems imaging platforms faxes and electronic medical records.

Create AM/PM Daily Sheet: Perform pre-authorizations and insurance verification for in-network transferring patients. Other duties as assigned. Promote the use of image retrieval systems to coordinate patients radiologic studies while in the transfer process.

Monitor Daily: Teams all emergent and non-emergent cases pending cases for cancellation phones fax server email platform Pending requests Accepted requests Ground/Air Dashboard

LVNs may answer the phone to facilitate a non-emergent transfer request.
