POSITION SUMMARY
- The Home Health Director is responsible for overseeing all aspects of home health operations ensuring the delivery of high-quality patient care and compliance with company and regulatory standards for multiple business lines including Medicare Part A Medicare Part B and Assisted Living.
- This leadership role involves strategic planning program development multidisciplinary collaboration and staff management to enhance service delivery and optimize patient outcomes.
- The ideal candidate will have extensive experience in home health care strong leadership skills and a commitment to patient-centered care.
DUTIES AND RESPONSIBILITIES

Operational Leadership:
- Oversee daily operations of home health and assisted living services ensuring compliance with state and federal regulations.
- Develop and implement policies and procedures to improve operational efficiency and quality of care.
- Collaborate with Management Clinical Directors and Talent Acquisition to share best practices discuss innovations and foster teamwork to enhance service delivery.
Program Development:
- Design and implement Care Cycle Coordinator (CCC) programs tailored to meet the needs of diverse patient populations broken down by territory.
- Evaluate program effectiveness and make adjustments based on feedback and outcomes.
- Develop and implement a scheduling system that will increase service units through optimizing scheduling and resource allocation amongst the CCCs to increase service units.
Clinical Oversight:
- Have a concrete understanding of clinical verbiage and ability to implement clinical standards within the Home Health Department.
Staff Management:
- Recruit train and supervise CCCs fostering a culture of teamwork and continuous improvement and providing professional development.
- Collaborate with multidisciplinary teams to optimize patient care and hold weekly team meetings to review KPIs set provide feedback regarding potential improvements in current process and address any gaps in healthcare services.
- Set specific measurable achievable relevant and time-bound (SMART) goals for CCCs to achieve and provide checklists to follow and ensure success in the process.
Quality Assurance:
- Monitor patient care quality and satisfaction implementing quality improvement initiatives as necessary.
- Conduct regular audits and assessments to ensure compliance with company as well as health and safety regulations.
Financial Management:
- Develop and manage budgets for home health operations ensuring cost-effective operations.
- Analyze financial performance and implement strategies for revenue enhancement and cost control for billable units.
Stakeholder Engagement:
- Build and maintain the relationships between patients families healthcare providers and other relevant stakeholders to ensure clear communication and coordinated care.
Reporting and Analysis:
- Prepare regular reports on operational performance patient outcomes and service quality for senior management.
- Utilize software to track KPIs and workflows for each CCC to ensure efficiency.
- Utilize data analytics to drive decision-making and improve service delivery.
- Ability to analyze cost against revenue to determine and measure the companys return on investment.
Qualifications
- Bachelors degree in healthcare administration nursing or related field; Masters preferred
- Proficient in Microsoft Office
- 5 years of experience in home health care management or similar role
- Strong knowledge of healthcare regulations quality assurance and clinical practices
- Excellent leadership communication and interpersonal skills
- Proficiency in electronic health record (EHR) and healthcare management software
- Strong understanding of patient confidentiality and privacy laws
- Compassionate and empathetic approach to patient care
- Ability to work in and foster a team environment
- Concrete understanding of cost versus revenue and ability to analyze the data to implement plans as needed.
