Job Description

Purpose

The Health Project Supervisor MBCCAP is responsible foroverseeing the Milwaukee Breast and Cervical Cancer Awareness Program (MBCCAP)and the Wisconsin Well Woman Program (WWWP) as well as for coordinating withother governmental and community organizations for womens health services.

Essential Functions

PROJECT OVERSIGHT

CLIENT SERVICES AND COMMUNITY COLLABORATION

  • Collaborate with community agencies health providers and clinics to enhance enrollment in the MBCCAP and WWWP.
  • Ensure that community-based case management providers maintain appropriate case management documentation data collection and billing practices.
  • Collaborate with local providers on ongoing program planning and updates; monitor cases.
  • Establish develop and maintain referral relationships with entities involved in case management and service to the target population such as health providers hospitals and health clinics.
  • Coordinate continuum of care efforts with other Community-Based Organizations to identify available resources that meet the needs of targeted families; develop and maintain a manual of client-based community resources.
  • Coordinate meetings outreach and education with community agencies health clinics hospital systems public and private entities to increase awareness and patient screenings.

SUPERVISION

  • Provide programmatic supervision and evaluation of Program Assistants Community Education Assistants Medical Assistants Public Health Nurse Leaders and Public Health Educators.
  • Supervise assign and support a team of professionals in providing client-based case management and home visiting services.
  • Provide orientation coaching mentoring guidance supervision and evaluation of staff to assure safe practices and quality of services; recommend personnel actions including discipline.
  • Ensure that staff community partners and providers comply with programmatic guidelines and initiate necessary changes to improve processes.
  • Represent the MHD at meetings committees programs and in community efforts.
  • Ensure that staff are trained on all relevant protocols processes and policies providing them with guidance and direction.
  • Participate in Emergency Preparedness and Safety Culture and Wellbeing initiatives and activities.

Conditions of Employment

Minimum Requirements

  1. Bachelors degree in health education public health nursing social work or a related field from an accredited college or university.
  2. Two years of progressively responsible experience coordinating public or community health programs; experience may include community organizing or collaboration health program planning development implementation and/or evaluation.
  3. Valid drivers license and possession of a properly insured vehicle for use on the job is required at time of appointment and throughout employment. Private auto allowance may be paid pursuant to Section 350-183 of the Milwaukee Code.

Equivalent combinations of education and experience may alsobe considered.

Current Salary

The current salary range (Pay Range 1GX) is$87656-$98703annuallyand the resident incentive salary range for City of Milwaukee residentsis$90286-$101664annually.Appointment above the minimum ispossible based upon level of experience and other qualifications and is subjectto approval.

THE SELECTION PROCESS will be job-related and will consist of one or more of the following: an evaluation of education experience and/or responses to supplemental questions; a written or performance test a structured interview or other assessment methods.
