Description

(FDDP) Front Door Diversion Program Supervisor

Location: Matteson IL

Compensation:$53000 to $57000

Schedule:Saturday to Wednesday 1-9 PM (1 weekend shift required)

Position Summary


The Front Door Diversion Program Supervisor makes a meaningful difference in the lives of individuals served by the National Youth Advocate Program. This position is responsible for the management quality and administration of programing and direct supervision for assigned staff. The (FDDP) Program Supervisor serves as a member of the management team ensuring strength based practices in programming according to the NYAP mission vision and values. The (FDDP) Program Supervisor will also ensure compliance to the programs contractual guidelines.


RESPONSIBILITIES


The Front Door Diversion Program Supervisor will perform duties including but not limited to:

  • Perform all work in a manner consistent with the National Youth Advocate Programs mission values and philosophies.
  • Provide consultation and advocacy on behalf of clients with hospitals community agencies government agencies andother providers in the community.
  • Assist in the coordination of all intakes initial paperwork and documentation at the FDDP transitional homes and/orassigned hospitals.
  • Assist in the referral and assessment process through Maximus Assessment Pro.
  • Conduct staff supervision for direct reports on a scheduled basis.
  • Assist in the development and implementation of training plans for team members.
  • Provide ongoing training and other team building activities in staff team meetings.
  • Maintain productivity standards monthly for direct reports.
  • Sign off on all Incident Reports Critical Incident Reports and other required reports as well as submitting the reports forprocessing according to agency policy.
  • Assist in maintaining house coverage in the various transitional homes.
  • Review and assure the completion of monthly and quarterly reports by direct reports.
  • Submit applicable weekly and monthly reports to the Program Director.
  • Perform all duties in accordance with personnel policies and the programs fiscal guidelines.
  • Assist Clinical Services Managers in quality assurance activities when needed.
  • Work collaboratively with other NYAP programs and staff throughout the organization.
  • Other duties as assigned by FDDP or NYAP leadership.
  • Provide on-call services to respond to after hour emergencies and crises based on program needs.
  • Attend team meetings and provide consultation and feedback to team members to assure high quality and effectiveservices.
  • Performs other duties as requested.


MINIMUM QUALIFICATIONS

  • Masters degree in social work or comparable Human Services field from an accredited institution.
  • Two years direct experience in the field of human services.
  • Relevant experience working with individuals with mental illness co-occurring disorders and/or experience homelessness.
  • Must be knowledgeable of group facilitation/mediation practices and principles.
  • Training education and/or experience related to the following:
    • Conflict resolution
    • Crisis Intervention
    • Housing First Model
    • Mental Health awareness
    • Substance use and harm reduction model
    • Strengths based assessment and planning
    • Communication and ability to relate to diverse groups and manage interpersonal conflict
  • Proficient use of desktop and laptop computers smart phones and tablets printers fax machines and photocopiers as wellas software including word processing spreadsheet and database programs.



OTHER SKILLS

  • A valid LSW or LPC is preferred but not required.
  • Some travel is required.
  • Must have reliable transportation and a valid drivers license.
  • Must be open and willing to work non-traditional work hours as well as on-call responsibilities.
  • Must be able to embrace working as a diverse team both effectively and respectfully with persons of any background racecolor religion sexual orientation ability national origin or culture.
  • Provide excellent internal and external customer service.
  • Must possess good verbal listening and written communication skills.
  • The person in this position needs to follow a team concept and support both agency goals and co-workers. Employees must be ableto effectively work with and be respectful and sensitive to persons from various cultures socioeconomic ethnic religious and racialbackgrounds.
