Physical Therapist LTC/SNF - Travel Position
Location: Snow Hill NC
Job Type: Travel Contract
Duration: 13 weeks
Shift: 5x8s Days
Pay: $2035/week
Job Description:
We are seeking a dedicated and compassionate Physical Therapist to join a skilled nursing and long-term care facility in Snow Hill NC on a travel contract. The Physical Therapist will evaluate residents design individualized treatment programs and provide therapy services that promote mobility restore function and enhance independence. The ideal candidate is patient-focused detail-oriented and experienced in geriatric rehabilitation.
Key Responsibilities:
- Conduct comprehensive evaluations to determine mobility strength and therapy needs.
- Develop and implement individualized treatment plans in coordination with physicians and care teams.
- Deliver therapeutic interventions including strengthening gait training balance exercises and mobility support.
- Educate residents families and caregivers on therapy goals safe practices and assistive equipment.
- Document evaluations progress notes and treatment outcomes accurately and timely.
- Collaborate with interdisciplinary staff to ensure coordinated patient-centered care.
- Promote safety independence and quality of life through evidence-based therapy services.
Qualifications:
- Doctor of Physical Therapy (DPT) or Masters in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in North Carolina (or compact license).
- Minimum of 1 year of recent PT experience preferably in skilled nursing or long-term care.
- Current certification in Basic Life Support.
- COVID-19 vaccine and recent tuberculosis test.
- Strong communication assessment and organizational skills.
- Compassionate approach with experience supporting geriatric populations.
