SUMMARY OF DUTIES

The primary purpose of this position is to assess the physical and cognitive needs of Oregonians applying for or currently receiving long-term care services. The position also determines some financial eligibility for some long-term care programs. It is responsible for developing coordinating and arranging a variety of services supporting older adults and people with disabilities in receiving continuity of care. It also allows families in Oregons family assistance programs to become self-sufficient while helping them meet their basic needs.

Case Managers help develop and implement case management plans coordinate the provision of services for community-based care and nursing home care regularly review and update long-term care service eligibility monitor social service plans and identify and mitigate safety risks.

MINIMUM QUALIFICATIONS

A Bachelors degree in a Behavioral Science Social Science or a closely related field;

OR

A Bachelors degree in any field and one year of human services related experience (i.e. work providing assistance to individuals and groups with issues such as economically disadvantaged employment abuse and neglect substance abuse aging disabilities prevention health cultural competencies inadequate housing);

OR

An Associates degree in a Behavioral Science Social Science or a closely related field AND two years of human services related experience (i.e. work providing assistance to individuals and groups with issues such as economically disadvantaged employment abuse and neglect substance abuse aging disabilities prevention health cultural competencies inadequate housing);

OR

Three years of human services related experience (i.e. work providing assistance to individuals and groups with issues such as economically disadvantaged employment abuse and neglect substance abuse aging disabilities prevention health cultural competencies inadequate housing).

ESSENTIAL ATTRIBUTES

*Preference will be given to candidates that are bilingual in Spanish.
