We are a CQC-registered care provider supporting individuals with mental health needs physical disabilities and complex care requirements across community settings supported living and residential care. To help us continue to provide outstanding services we are seeking a dedicated Care Coordinator to join our team.

Role Purpose

The Care Coordinator will be responsible for managing and coordinating care packages ensuring staff are effectively deployed and maintaining compliance with CQC standards. This role is key to ensuring service users receive safe consistent and high-quality care tailored to their individual needs.

Key Responsibilities

  • Coordinate care services in line with individual care plans ensuring continuity and quality.

  • Develop review and update care plans with input from service users families and professionals.

  • Schedule and allocate staff to meet care package requirements ensuring safe staffing levels.

  • Support the Registered Manager in meeting CQC compliance and regulatory requirements.

  • Monitor service delivery carry out audits and address any issues promptly.

  • Act as a point of contact for service users families staff and external professionals.

  • Support with staff training supervision and competency checks where needed.

  • Participate in on-call rota to support service continuity.

Person Specification

Essential

  • Experience in coordinating or delivering care services within a CQC-regulated environment.

  • Strong knowledge of care planning safeguarding and compliance frameworks.

  • Excellent organisational and scheduling skills.

  • Strong communication and interpersonal skills with the ability to build positive relationships.

  • Ability to work under pressure and adapt to changing priorities.

  • IT literacy including care management systems.

Desirable

  • NVQ Level 3 in Health & Social Care (or working towards).

  • Previous experience in a care coordination role.

  • Experience supporting individuals with mental health needs physical disabilities or complex care.
