The Opportunity Alliance seeking aFull Time Residential Nurse (RN) or Licensed Practical Nurse (LPN)to support ourOcean Street adult residential program.

The Ocean Street Residential Program with a capacity of eight residents provides trauma-informed treatment and support services to adults living with mental illness complicated by co-occurring chronic health conditions as well as a potential substance use a supportive safe and secure environment the program meets the needs of individuals who have a major mental illness by providing psychosocial rehabilitation services and personal care services.

The Residential Nurse/ LPN contributes to the accomplishment of the Agencys mission through the evaluation of medication management practices other pertinent health education programming direct care of client medical needs as appropriate to the scope of a RN or LPN care coordination of physical health needs with medical providers as well as the shared oversight of the health and well-being of clients within Residential Treatment Services.

This position is a member of the Adult Residential Treatment Team. Residential Treatment serves adults living with serious and persistent mental illness substance misuse and may be experiencing homelessness. The Residential Nurse will partner closely with the Director to achieve positive program and client outcomes. The Residential Nurse will work within an interdisciplinary team approach to support integrated health outcomes for clients with mental illness complicated by core morbid chronic health conditions who may also have a substance use disorder. The Residential Nurse will utilize the recovery oriented trauma-informed and person-centered approach to care.

Primary responsibilities:

  • Providing direct nursing care within the scope of RN practice and as medically ordered for clients in the residential program;
  • Support integrated physical health care assist with admission and discharge providing care transition planning and care coordination and monitor medication procedures
  • Contributing to the achievement of positive client outcomes and results by working closely with Residential Director and developing wellness programming
  • Providing training teaching coaching instruction consultation and health education for residential staff that contributes to the achievement of positive client outcomes

Schedule: This is a full-time 40-hours/week position

Qualifications:

  • Licensed practical nurse holding a valid license to practice nursing in the State of Maine and license must be in good standing are required; with two (2) to four (4) years of experience required with recommendation that one of those years being in clinical practice in metal health nursing orRegistered Nurse holding a valid license to practice nursing in the State of Maine and license must be in good standing are required; with two (2) to four (4) years of experience required with one of those years being in clinical practice in metal health nursing.
  • Previous training experience in care coordination and/or integrated care is preferred.
  • Experience within the Mental Health and Substance Use delivery system and/or direct service is preferred.
  • Experience in the education and clinical care of clients with diabetes is a plus.
  • Advanced computer skills in programs including Microsoft Word Excel and PowerPoint required.
  • Strong written and oral communication and interpersonal skills required
  • Strong attention to detail required.
  • Experience in team or multidisciplinary approach to service planning is preferred.
  • Must be able to able to successfully pass a criminal background child protective service check & sex offender check and must not be on the state or federal suspension and disbarment list.
  • Must be able to exert moderate physical effort with occasional standing walking reaching stooping bending kneeling crouching typing and frequently lift and carry material weighing up to 50 pounds.
  • Driving may be necessary to attend meetings and trainings and for the occasional need to meet with clients offsite and within the program service area. This may include transportation of clients or driving to agency location to complete work. A good driving record is required.
