Job Description

An organization is seeking a warm dedicated and people-oriented Childrens Care Manager to support families and help them access government programs and services. This role carries a caseload of approximately 30 families providing both administrative and advocacy support. There is an opportunity for growth into a Care Supervisor role overseeing and mentoring care managers while continuing to carry a caseload.

Responsibilities Include:

  • Managing a caseload of about 30 families

  • Supporting families in accessing Medicaid OPWDD home modifications and other government programs

  • Handling documentation paperwork and compliance requirements

  • Building and maintaining strong relationships with families and agency contacts

  • Advocating for families and helping them navigate government systems

  • As a Care Supervisor: mentoring care managers setting goals and ensuring quality service delivery

Ideal Qualifications:

  • Bachelors degree required

  • Familiarity with Medicaid OPWDD or other government programs preferred

  • Strong communication and organizational skills

  • Compassionate patient and relationship-oriented

  • Must have a valid drivers license and car for travel across Long Island
