Job Summary

An organization is seeking a warm dedicated and people-oriented Childrens Care Manager to support families and help them access government programs and services. This role carries a caseload of approximately 30 families providing both administrative and advocacy support. There is an opportunity for growth into a Care Supervisor role overseeing and mentoring care managers while continuing to carry a caseload.

Responsibilities Include:

Managing a caseload of about 30 families
Supporting families in accessing Medicaid OPWDD home modifications and other government programs
Handling documentation paperwork and compliance requirements
Building and maintaining strong relationships with families and agency contacts
Advocating for families and helping them navigate government systems
As a Care Supervisor: mentoring care managers setting goals and ensuring quality service delivery

Ideal Qualifications:

Bachelors degree required
Familiarity with Medicaid OPWDD or other government programs preferred
Strong communication and organizational skills
Compassionate patient and relationship-oriented
Must have a valid drivers license and car for travel across Long Island
