Job Summary

Canadian Mental Health Association Peel Dufferin (CMHA Peel Dufferin) is pleased to partner with various agencies in the Peel Region to offer services to the local community at the *new* Brampton based Homeless and Addictions Recovery Treatment (HART) Hub.

HART Hubs are community-based care hubs designed to address mental health addiction and housing needs for people experiencing homelessness. They provide integrated care offering a wide range of services such as primary care mental health services addiction care and employment supports. With targeted community outreach and rapid referral pathways HART Hubs aim to support individuals with complex needs helping them recover achieve housing stability and inclusion.

We are looking for a full-time permanent Team Lead to join our team!

Supporting a multi-disciplinary team the Team Lead provides support in the day-to-day operations and clinical delivery of core services while ensuring the provision of evidence-based care while fostering a collaborative team environment.


Status: Permanent full-time 12 hour shifts on a rotational schedule between hours of 9am-10pm Monday through Sunday. Note this posting is for an active vacancy.
Salary: $74028.74 - $87411.79 annually. (Candidate qualifications will be considered when determining the starting salary)
Location:This position is based out of our Brampton office. Due to the nature of this program this role is on-site and/or community based.


What You Will Do:

  • Provide direct clinical interventions with HART Hub clients to promote community integration care planning and symptom recovery
  • Conduct regular individual and group clinical supervision with the team
  • Support staff in prioritizing client issues and tasks as needed
  • Act as back-up to the HART Hub Program Manager as needed
  • Support the onboarding and training of all staff including identifying learning and development opportunities
  • Work in collaboration with the HART Hub Program Manager to support the implementation of programs and services using your clinical judgement and expertise to navigate matters of a clinical issue to ensure optimal client/family care and provider experience
  • Support the evaluation of existing programs and work with the team to identify and implement improvements
  • Lead team huddles and meetings
  • Ensure adequate staff coverage during shifts
  • Assessing clients to access specific staff and/or programs as appropriate
  • Identify risks and/or performance issues and escalate them to the HART Hub Program Manager
  • Maintain and/or develop relationships with external networks related professional organizations and other related service providers to support organizational goals

What You Need to Join Our Team:

  • University degree or college diploma in Social Work Psychology Psychotherapy Nursing or related discipline
  • Minimum five years experience in community mental health and addictions setting
  • Minimum one year of supervision or leadership experience in a clinical or community setting
  • Registration and in good standing or in the process of registering with appropriate College or Professional association
  • Demonstrated experience working within a multidisciplinary team and evidence of an ability to collaborate and partner
  • Experience working with individuals experiencing addiction and homelessness challenges is an asset
  • Working knowledge and/or an understanding of various disciplines; including but not exclusive of mental health substance use harm reduction newcomer challenges and barriers faced by those in marginalized populations
    • Fluency in a second language including ASL (American Sign Language) is considered an asset
    • Demonstrated cultural competence ability to work and apply practical understanding of different elements of diversity equity inclusion and belonging as well as an understanding of social determinants of health and impact on vulnerable communities
    • Experienced at engaging in matters that require sensitivity and empathy across a diverse community excellent judgement and discretion; ability to operate with various frameworks including culturally sensitive anti-racist anti-oppressive 2SLGBTQ affirming and intersectional
    • Ability to work 12 hour shifts on a rotational schedule: days evenings weekends and public holidays
    • Valid Canadian drivers license and access to an insured automobile in good repair
    • A satisfactory Vulnerable Sector Screening (Police Check) report
