POSITION SUMMARY
St. Vincent de Paul Georgia (SVdP) is dedicated to supporting individuals and families experiencing homelessness or housing instability in their journey toward stable sustainable housing. This new position that will manage our pilot STABLE (Supporting Transition to a Better Living Environment) Programan initiative that combines Transitional Housingand Rapid Rehousing services.
The STABLE Program Manager will be responsible for the successful implementation and day-to-day management of this exciting and impactful program which will serve 1218 households over an 18-month period. This role requires strong skills in program and project management budget oversight and case coordination to ensure that the program effectively supports participants as they transition from homelessness to permanent self-sufficient housing.
The program provides individualized case management as households move from a subsidized motel setting to a subsidized rental unit and ultimately into stable independent housing. This is an exciting opportunity for a passionate and organized housing professional to make a meaningful impact in the lives of those striving for housing stability. This is an 18 month grant funded program with the potential of transitioning to a regular full time position. While this is a remote position applicants must be in the Atlanta GA metro area and able to commute to Chamblee GA and Gwinnett County GA for meetings and to meet with program participants.
Job Responsibilities:
Program Manager Duties:
  • Develop a roadmap for program development in conjunction with Housing Director
  • Manage program budget ensuring maximum impact and efficient use of funds
  • Participates in quality improvement initiatives to enhance the program and client experience
  • Monitor participant progress and adjust program as necessary
  • Collect and analyze data from program participants to assess the pilots impact
  • Prepare reports for funders and leadership as requested
  • Use feedback to refine the program and inform future scaling decisions.
  • Stay informed about best practices.
  • Coordinates assistance to clients from volunteers
  • Provides internal and external presentations to stakeholders about the STABLE program.
  • Participate in regular team meetings and supervision sessions.
Intake/Case Management Related Duties:
  • Conducts initial pre-screen and assessment of clients to determine their needs and eligibility for services.
  • Provides clear information to clients about available services eligibility criteria and the intake process.
  • Gather and document pertinent information regarding the clients background and any other relevant details.
  • Maintain accurate and up-to-date records and case notes.
  • Maintain compliance with HMIS and VAWA requirements.
  • Create and monitor individualized service plans (ISP) in collaboration with program participants with time-sensitive goals for employment and housing.
  • Maintain consistent case manager check-ins to monitor ISPs
  • Coordinate with SVdPs LEAP (landlord engagement) IAP (income advancement) programs benefits navigator and other community-based services to connect clients with appropriate services and resources.
  • Recognize urgent or crisis situations and takes appropriate actions such as referring clients to emergency services or crisis teams.
  • Provide support and guidance to clients during challenging times.
Qualifications
  • Bachelors degree in social work psychology or a related field preferred.
  • Previous experience in case management in a housing or homelessness-related program; strong understanding of homelessness and housing resources in the metro area; familiarity with Gwinnett County resources strongly preferred.
  • Previous experience in workforce development preferred.
  • Knowledge of rapid rehousing transitional housing and related housing programs.
  • Excellent communication and interpersonal skills.
  • Excellent organizational skills.
  • Exhibits a strong desire to help individuals and families who have experienced housing insecurity.
  • Ability to work independently and as part of a team
  • Experience with trauma informed care preferred
  • Proficiency with data entry and case management software
  • A valid drivers license and vehicle are required as well as proof of auto insurance.
  • Ability to work remotely as well as attend in person meetings.
  • Ability to support the mission of a Catholic based non-profit organization
Compensation
Competitive salary commensurate with experience and background plus benefits including health insurance and 401K retirement plan 401K match STD LTD Life Insurance FSA.
