Job Description

Job Title: Pharmacist
Site: Phoenix Indian Medical Center
Location: 4212 N 16th Street Phoenix AZ 85016
Duration: 1 year contract with possibility of a 4 years extension

Shift Schedule:

  • Full-time 8-hour shifts (Monday Friday for most locations).
  • Phoenix and Whiteriver units may include evening night weekend or holiday shifts.
  • Example schedules: 8:00 a.m. 5:00 p.m. 9:00 a.m. 6:00 p.m. 1:00 p.m. 10:00 p.m. 3:00 p.m. 12:00 a.m.

License / Certifications:

  • Active unrestricted Pharmacist license (U.S. state/territory).
  • Current Basic Life Support (BLS) certification

Education:

  • Bachelors or Doctor of Pharmacy (PharmD) degree from an accredited program

Minimum Qualification / Experience:

  • At least 2 years of experience as a licensed Pharmacist.
  • Proficiency in EHR systems and documentation.
  • Strong communication and interpersonal skills to work with diverse patient populations

Responsibilities:

  • Review patient charts lab work allergies and vitals to verify medication orders.
  • Process fill and check prescription orders for accuracy.
  • Provide medication counseling and patient education.
  • Collaborate with providers and staff to ensure safe and effective therapy.
  • Manage controlled substances audits and IV admixtures.
  • Administer immunizations (with certification).
  • Answer provider and patient inquiries regarding drug therapy.
  • Recognize and respond to adverse drug reactions and emergencies.
  • Participate in mandatory training and competency assessments
