Description

The role of the Community Health Worker is to provide culturally appropriate health education and information and to assist patients with navigating the health care system. The CHW must be an advocate for marginalized and high-risk individuals with chronic medical conditions. S/He must feel comfortable engaging with individuals and families in their homes as well as in the health center. This is a full-time (40 hours per week) position.

Primary Responsibilities:

Patient Outreach

  • Conduct assessments of patient skills and areas of need
  • Coordinate social and medical services for patients
  • Provide home-based health promotion intervention to high-risk patients with chronic medical conditions
  • Deliver health promotion education
  • Support participants in achieving improved self-management skills medication adherence and adoption of healthier lifestyle choices

Clinical Liaison

  • Facilitate communication between patient and care teams
  • Coordinate with Primary Care Teams for patient referrals and follow up
  • Assist patients with health system navigation and accompany them to appointments as deemed necessary
  • Facilitate referrals to community resources

Reporting

  • Attend all trainings and meetings as needed
  • Document all patient visits and contacts in EMR and route to supervising RN and PCP
  • Discuss and document all patient self-management goals in EMR
  • Follow emergency protocols during patient contacts (i.e.: DAP)
  • Maintain caseload and documentation for grant reporting
  • Other duties and/or other locations as assigned

Qualifications

Qualifications and Skills:

  • Bachelors degree preferred or equivalent and relevant professional experience
  • Familiarity with diabetes hypertension or COPD and experience working with people with these conditions preferred
  • Bilingual fluency in English and Spanish or English and Haitian Kreyol preferred
  • Strong writing and documentation skills are required
  • Possession of a vehicle for transportation is required
  • Typing skills and computer literacy are required

Physical Requirements

  • Must be able to stand or sit for prolonged periods (50% of the time)
  • Must be able to lift 10-25 pounds and load onto shelves.
  • Visual acuity sufficient for frequent reading and computer use.
