Schedule: Full-Time 9:00 AM 5:00 PM (MondayFriday some weekends)
Salary: $90000 $115000 bonus potential
myLaurel delivers hospital-level care directly into patients homes improving outcomes preventing readmissions and creating a better experience for patients and caregivers. Our programs include Rapid Advanced Care Acute Care at Home and Recovery at Homeall supported by a team of telehealth physicians in-home paramedics and RN care managers.
We are hiring a Clinical Liaison (RN) to support the launch and growth of a new hospital-partnered service line. You will act as the face of myLaurel inside the hospital building trust identifying eligible patients and ensuring smooth transitions into our home-based acute care programs. This hybrid clinicalstrategic role is ideal for a nurse experienced in inpatient hospital care who excels in relationship-building and care coordination.
Build strong relationships with hospital staff physicians and leadership
Identify eligible patients and manage referrals into myLaurel programs
Educate patients and caregivers on services and care expectations
Collaborate with hospital staff to integrate workflows
Track referral activity patient outcomes and satisfaction
Support strategic growth of the new service line
Current unrestricted RN license
Floor Registered Nurses Only with hospital experience (ICU/ER preferred)
5 years of inpatient clinical experience
Strong care coordination discharge planning and communication skills
EHR proficiency; BSN or related degree preferred
Medical (choice of 6 Aetna plans) dental vision eligible after 1 month
Unlimited PTO after 90 days (taken in two-week buckets)
Travel reimbursement (parking only)
