Home Care Launceston / Shearwater
Permanent Part-Time (3 days per week)

About the role
We currently have a permanent part-time opportunity for a skilled enthusiastic and dedicated Commonwealth Home Support / DVA / VHC Care Case Manager to join our Home Care Team in Launceston willing to travel to Shearwater on a regular basis.
As a Commonwealth Home Support / DVA / VHC Care Case Manager you will have an opportunity to:
- Provide day to day management of a client case load care planning and provision of consumer directed services to clients
- Make a real difference to our consumers lives
- Enable our consumers to remain in our own homes
What you bring to the role
- Relevant experience in case management community services aged care or equivalent (Essential)
- Knowledge of CHSP and VHC funding(Essential)
- Experience in service delivery to clients living in the community
- High level planning and organisational skills
- Well-developed communication skills
- A desire to create a positive experience for our consumers
- A current unrestricted drivers license(Essential)
