Veteran Rehabilitation Case Manager

As the Veteran Rehabilitation Case Manager you will provide continuity of care across the spectrum of mental health services for U.S. Armed Forces service members/veterans/veteran families. Specific referrals and resources are based on Active Duty/Reserve/Veteran status of individual(s) served.
Minimum qualifications:
Minimum Education: Bachelors Degree
Defined Education: Social behavioral health and human services or nursing
License/Certifications:Valid Texas Drivers License and proof of Liability Insurance Coverage
Qualified Mental Health Professional Community Support (QMHP-CS) credentialing
Special Courses: Successful completion of required organizational training
