Position Overview
We are seeking an experienced and compassionate Home Health Manager (RN) to lead a high-performing home care department. The ideal candidate will oversee nursing staff ensure compliance with workflows and policies and maintain high standards of patient care for elderly and frail participants. This role requires a hands-on leader with strong clinical expertise and management skills capable of fostering a collaborative and supportive team environment.
Key Responsibilities

Direct and manage the home care department ensuring all workflows and policies are consistently followed.

Lead train coach and support home care staff including hiring and onboarding new team members.

Assess the effectiveness of home health nursing assessments and interdisciplinary team (IDT) participation.

Ensure nursing interventions are delivered and accurately documented according to participants care plans.

Monitor and maintain the quality of care provided to participants in nursing homes and board-and-care facilities.

Implement and maintain best practices in home health care and regulatory compliance.
Qualifications & Requirements

Bachelors Degree preferred; additional years of relevant experience may be considered in lieu of a degree.

Minimum of 2 years of nursing experience in a home health setting preferably working with frail or elderly populations.

California Registered Nurse (RN) License required.

Strong clinical knowledge to manage complex patient scenarios and ensure high-quality care.

Demonstrated leadership skills with experience managing interdisciplinary teams.
