Your Opportunity

The Universal Aftercare Service is a non-clinical support service focused on providing rapid practical psychosocial support to people experiencing a suicidal crisis or who have attempted suicide.

Were looking for passionate and driven Care Coordinators to join this exciting new service designed to reach and support communities who may not typically engage with the health system.

This program will deliver intensive specialised support across the Shoalhaven and Goulburn regions with a strong focus on engaging men including Aboriginal men through culturally safe and responsive services.

Our Care Coordinators work alongside our Peer Workers to support people experiencing suicidal crisis and/or distress. Working within a recovery framework in collaboration with other relevant services they support people to build independence in their daily life and contribute to their recovery community integration and healing.

Your key responsibilities will be to:

  • Undertake initial intake and safety planning for clients and work with individuals and their families to create individualized case plans.
  • Provide ongoing support sessions with clients and review progression and safety as required.
  • Support and promote family carers friends and kin involvement in safety and support planning in collaboration with clients
  • Liaise with other support services and organisations to coordinate the requirements of the case plan.
  • Deliver group programs which support the practical needs of the clients
  • Assist clients in the process of transition out of the service into independence or other services.
  • Ensure documentation is completed and updated in line with Mission Australia protocols.

Requirements for Success

  • Relevant tertiary qualifications and/or significant relevant work experience
  • A sound understanding of mental health and suicidality mental health systems and recovery-oriented practice as well as psycho-social rehabilitation.
  • Experience working within a peer workforce with a focus on trauma-informed recovery-oriented practice and valuing lived experience
  • A sound knowledge of crisis intervention services and their functions within the region and steps to engage individuals into these services.
  • Case management experience
  • Computer literacy preferably using Windows.
  • Current NSW Drivers License
  • Current First Aid Certificate

Before starting work with us you will need to undertake a national police history check qualifications and referee checks and have a clearance to work with children/vulnerable people.
