JOB SUMMARY:
Under the general supervision of the PTW Health Home Supervisor engagement specialist provides Care Coordination and Outreach services for Children and Adults. The engagement specialist is responsible for outreach and enrollments to the program and ensuring that the clients are connected to the appropriate services. Quality Improvement functions include reviewing eligibility and appropriateness. Ensuring that all program documentation is complete and submitted in a timely manner. Establishes and maintains systems to ensure that programs meet or exceed agency and regulatory guidelines and that documentation and billing conform to all applicable targets rules and regulations.

1. Conducts proactive outreach to eligible individuals through phone calls home visits community events and provider referrals to encourage enrollment in the program.
2. Build trust and rapport with members and their families by actively listening addressing concerns and explaining program benefits in a clear and supportive manner.
3. Participates in community fairs health events and organizational outreach activities to raise awareness and increase program participation.
4. Completes initial intake activities gathers consent verify demographic and Medicaid information and confirms eligibility for enrollment.
5. Engage members in discussions about their needs strengths and personal goals to begin the foundation of a care plan.
6. Works collaboratively with members to explore and identify preliminary health and social goals that align with their needs and preferences.
7. Provides education on available resources and support ensuring members understand how the program can help meet their goals.
8. Encourages and motivates members to actively participate in their care fostering self-advocacy and empowerment.
9. Assists in developing a warm handoff to the assigned Care Coordinator ensuring continuity and a smooth transition.
10. Participate in initial case discussions with Care Coordinators to ensure they receive accurate background goals and rapport established with the members.
11. Completes timely accurate documentation of outreach activities intake information and engagement notes in accordance with program and regulatory requirements.
12. Tracks outreach outcomes enrollment progress and member engagement efforts for reporting and quality improvement.
13. Immediately reports incidents allegations of abuse or neglect or other sensitive situations in accordance with agency and regulatory policies.
14. Maintains caseload until vacancy reported to transfer to teams following enrollment.

ESSENTIAL KNOWLEDGE SKILLS AND ABILITIES:
1. Committed to the active promotion of the ICL goals and values.
2. Good knowledge of the resources available in the community to support treatment plan goals.
3. Good knowledge of the components of a comprehensive Care Plan.
4. Ability to effectively use electronic health record software and other technology required to perform the duties of the job satisfactorily
5. Good knowledge of the policies and organization of the regulatory agency and applicable legislation regulations and rules.
6. Good knowledge of the principles and practices of supervision.
7. Basic knowledge of best practices evidence-based treatment and general treatment techniques.
8. Ability to plan and organize treatment services and to provide supervision and leadership to a culturally diverse group of staff.
9. Ability to assist in the preparation of written proposals for new or expanded treatment services.
10. Ability to identify from the record and from observation deficiencies in individual treatment plans.
11. Excellent oral and written communication skills.
12. Ability to efficiently and effectively manage resources including money material time and people
QUALIFICATIONS AND EXPERIENCE:
High school diploma or GED required plus two years providing direct services or in linking people with serious mental illness developmental disabilities or substance use disorders to a broad range of services essential to successful living in a community setting (e.g. medical psychiatric social educational legal housing and financial services). Bachelors degree preferred.
