Position Overview:
We are seeking a Registered Nurse (RN) to conduct Start of Care (SOC) assessments and 60/90-day reassessments for our clients. This is an independent contractor (W9) position offering flexible scheduling and rewarding work that directly impacts the well-being of those in need.
Responsibilities:
Conduct in-home Start of Care (SOC) assessments for new clients.
Complete 60-day and 90-day reassessments as required.
Develop and review care plans to ensure appropriate non-medical support services.
Communicate with caregivers and office staff regarding client needs.
Maintain accurate and timely documentation.
Requirements:
Active Registered Nurse (RN) license in the state of Ohio.
Experience in home care geriatrics or community health is a plus.
Ability to work independently and travel within the Cleveland area.
Strong assessment documentation and communication skills.
Reliable transportation and a valid drivers license.
Must be comfortable working as a W9 independent contractor.
Compensation & Benefits:
$40 - $75 per assessment based on assessment type.
$0.75 per mile reimbursement for travel.
