GENERAL RESPONSIBILITIES:

Reports to the Behavioral Health Program Manager. As a member of a multidisciplinary team of behavioral health providers the Case Manager will be responsible for the provision of case management and social services referrals to clients with substance use disorders (SUD) mild to moderate mental health disorders and/or co-occurring disorders (COD) who exhibit behaviors that increase their risks for HIV and hepatitis C (HCV) exposure. The incumbent will assist clients in establishing a supportive health network in a non-residential outpatient setting. The case manager will advocate for clients including but not limited to: drug and alcohol treatment and recovery services; mental health therapy; linkages to primary health care services; HIV hepatitis and STI screening testing and immunization services; and active referrals to education childcare housing vocational guidance employment health insurance navigation and financial entitlements resources. Ancillary duties include attending weekly supervisory sessions periodic staff meetings and required trainings and conferences; and other duties as assigned.

Duties

  • Conduct comprehensive intake assessments to identify client needs strengths and barriers related to health housing behavioral health and social services.
  • Develop individualized service plans (ISPs) in collaboration with clients and interdisciplinary team members ensuring they are client-centered measurable and aligned with ERCs program goals.
  • Monitor client progress toward service plan goals updating plans as needed documenting all services and contracts in accordance with agency and funding requirements.
  • Conduct risk reduction counseling and health education related to HIV HCV STIs mental health and substance abuse prevention and treatment.
  • Work as part of a fully integrated multidisciplinary team of behavioral health and medical providers to support clients with substance use (SUD) and/or co-occurring disorders (COD).
  • Perform comprehensive intake assessments and ongoing case management functions.
  • Enroll engage and link clients to appropriate medical care behavioral health services and social support programs.
  • Assist clients with developing and updating Individual Services Plans (ISP) and coordinate HIV/HCV testing preventions and treatment services.
  • Provide patient navigation and assisting with transportation coordination when needed.
  • Field outreach to support engagement and retention in care and conduct home visit if necessary if assigned by grants.
  • Address social determinants of health (housing food insecurity employment benefits) by making active referrals to ensure client become self-sufficient.
  • Maintain client documentation in the Electronic Health Record (EHR) in an accurate timely and compliant manner ensuring readiness for audits and grant reporting.
  • Collaborate with the interdisciplinary team during case conferences and care plan reviews to support holistic patient-centered care.
  • Provide regular follow-up interactions to bridge services gaps and ensure continuity of care including outreach to clients lost to follow-up.
  • Assist with monitoring program performance against grant deliverables and provide date for required funder reports.
  • Educate clients on available health programs insurance enrollment and medication assistance to reduce barriers to care.
  • Support quality improvement initiatives including tracking outcomes and identifying opportunities for services enhancement.
  • Adhere to NASW code of Ethics HIPPA regulations and maintain strict patient confidentiality.

Employment Standard & Conditions

EXPERIENCE: Bachelors Degree in a Human Services field required. Masters Degree in Social Work or Counseling preferred with a minimum of two years of case management experience and/or lived addition the incumbent must have a minimum of two years of experience with linkages to primary medical care mental health therapy and/or substance abuse treatment services. Experience working in a drug treatment facility is a plus. CASAC is desired.

  • Must be able to work as an integral part of the BH team and exhibit team building skills and competencies.
  • Must be extremely familiar with social services resources offered in the Atlanta Metropolitan Area and across the State of Georgia.
  • Must possess outstanding organizational time management and communication skills.
  • Must demonstrate solid decision-making skills.
  • Must have strong interpersonal skills and possess the ability to interact effectively among a multifaceted multidisciplinary team.
  • Must communicate effectively in writing as appropriate to document and record client service plans goals and progress.
  • Must have excellent oral and presentation skills to facilitate access to services and convey information effectively with staff clients physicians behavioral specialists and other service providers.
  • Demonstrated experience working with diverse medically underserved high risk populations.
  • Must be culturally competent.
  • Must have strong computer skills.
  • Must be able to work flexible hours when required.

Computer Skills:

  • Microsoft Office & other software: Intermediate to advanced knowledge of Access Word Excel PowerPoint Publisher Adobe.
  • Electronic mail software Email software; Microsoft Exchange.
  • Experience with utilizing an electronic health/medical records system preferred.

Hiring Salary Range:Negotiable. Salary commensurate with experience and qualifications.

Status:Full-Time.

Job Location:The position is based in Atlanta and requires work at the ERC office(s).
