About PACE
Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality affordable health and support services that preserve and protect their dignity quality of life and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly) affiliated with West Health () is a non-profit that delivers medical social and behavioral services to the frail elderly to the North County of San Diego. This PACE center is entrepreneurial in nature and focuses on innovations and bringing new processes to the traditional PACE environment.

Position Summary:
- The Gary and Mary West PACE (GMWP) Social Worker Supervisor provides leadership oversight and support to the Social Work team within the PACE program. This role ensures that participants and families receive high-quality person-centered psychosocial care in alignment with PACE philosophy and regulatory requirements. The Supervisor is responsible for staff supervision program development compliance monitoring and interdisciplinary collaboration to promote participant well-being independence and quality of life.
Essential Functions:
- Assists with selecting new members of the social services team including Social Workers Social Work Coordinator and interns as applicable and provides orientation to them.
- Supervises the social services team to ensure care coordination across disciplines providing guidance and direction training performance evaluation initial/annual competency and initiating corrective action as needed.
- Supports staff in developing implementing and monitoring individualized care plans in collaboration with the Interdisciplinary Team (IDT).
- Facilitates staff development through mentoring training performance feedback and continuing education opportunities.
- Participates in the intake and evaluation process as needed by conducting the initial social work assessment administering MSQs developing an intake summary and presenting the social work perspective to the Enrollment Huddle Team.
- Monitors and ensure compliance with PACE regulations state and federal guidelines and organizational policies related to social services.
- Assists with participant and family concerns serving as a resource for conflict resolution crisis intervention and care coordination.
- Participates in IDT meetings care planning and quality improvement initiatives.
- Maintains documentation standards and oversee timely accurate and complete social work records.
- Assures that an initial 30-day social service plan of care is developed and implemented for each new participant and updated based on the results of reassessments or on-going changes in status.
- Collaborates with leadership to assess department needs allocate resources and ensure adequate staffing.
- Contributes to program development and implementation of best practices in participant care including advanced care planning behavioral health support and caregiver support.
- Ensure culturally competent and trauma-informed practices in service delivery.
- Acts as liaison between the participant and other agencies such as Department of Aging Social Security Administration Medicaid etc.
- Assists with ongoing financial eligibility for participants including recertification as needed.
- Participates in participant-related conferences in the community as designated.
- Participates in the development promotion and implementation of caregiver support groups or programs; leads or co-leads groups as necessary.
- Maintains confidentiality of participant information.
- Participates in the development and implementation of QI activities.
- Attends and participate in all staff meetings in-services projects and committees as assigned.
- Pursues relevant professional development. Attends appropriate workshops and in-service activities to maintain current professional status
- Adhere to and support the West PACE policies practices and procedures.
- Performs other duties as assigned to support participant care and program operations.
- Accepts assigned duties in a cooperative manner; and perform all other related duties as assigned.
- Be flexible in schedule of hours worked.
- May require use of personal vehicle.
Education and Certification:
- Licensed Clinical Social Worker required.
- Current Basic Life Support (BLS) certification required.
- Original valid drivers license with proof of auto insurance.
Skills and Experience:
- Minimum of one (1) year of documented experience in working with a frail or elderly population required.
- At least three (3) years of previous management experience required preferably in a community-based or geriatric program.
- Demonstrates ability to supervise and direct professional personnel.
- Strong leadership communication and team-building skills.
- Knowledge of psychosocial needs of frail elderly people individuals with complex medical conditions and their families.
- Ability to work collaboratively with an interdisciplinary team in a fast-paced environment.
- Skilled in conflict resolution crisis intervention and problem-solving.
- Familiarity with regulatory requirements governing PACE programs and social services.
- Competency in documentation electronic health records and reporting.
- Commitment to the mission and values of PACE: Participant-centered dignity and quality of life.
- Bilingual English /Spanish highly preferred.
Medical Clearance:
Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. Job offers are contingent upon a successful pre-employment drug screen background check and physical assessment.
