Learn more about us by visiting: are hiring a Case Manager for both our Rapid Access Psychiatry (RAP) and Clinical Therapy teams! The Case Manager will provide brief case management and co-facilitate group sessions.

Closing Date: September 10 2025

Position: Permanent part-time working 21 hours a week; evening hours and weekends may be required.

Salary: $31.01-$33.22 per hour.*This represents the first three steps of a seven-step salary band. Candidate experience to be considered when deciding starting salary.

Location:

The successful candidate will work in a hybrid model and will be required to work out of the Aurora Office located at 15150 Yonge St Aurora ON L4G 1M2. The candidate will be expected to work on-site a minimum of 2 days per week and will be required to travel extensively within York Region & South Simcoe for client meetings staff training and meetings.

Program Profiles:

Rapid Access Psychiatry (RAP) is an evidence-based model of service for individuals ages 16 with mild-to-moderate depression and anxiety. The program provides individuals with an opportunity to attend a single appointment psychiatric consultation followed by a report with recommendations for treatment.

The Clinical Therapy Program is a group-based program that helps people with serious mental health issues to be more successful in day-to-day living by utilizing different forms of psychotherapy to help clients explore and manage unhelpful thoughts overwhelming/difficult emotions and problematic behaviours with the support of our team of Clinical Therapists. Our program helps clients develop and practice coping skills that help them gain control over unhelpful thinking patterns overwhelming/difficult emotions and unhelpful behaviours.

Job Profiles:

The Case Manager works with clients 16 years of age and older who are experiencing significant difficulties in day-to-day and role functioning as a result of serious and persistent mental illness and/or addictions. The Case Manager collaborates with the client to identify strengths and needs and develop long-term goals short-term objectives to meet these goals and concrete action steps to achieve the objectives. As part of this process individuals are assisted to acquire and use skills and to connect with formal and informal supports peer support professional services and resources to promote their recovery improve their quality of life and reduce the likelihood of relapse.
