Learn more about us by visiting: are hiring a Case Manager for both our Rapid Access Psychiatry (RAP) and Clinical Therapy teams! The Case Manager will provide brief case management and co-facilitate group sessions.
Closing Date: September 10 2025
Position: Permanent part-time working 21 hours a week; evening hours and weekends may be required.
Salary: $31.01-$33.22 per hour.*This represents the first three steps of a seven-step salary band. Candidate experience to be considered when deciding starting salary.

Location:
The successful candidate will work in a hybrid model and will be required to work out of the Aurora Office located at 15150 Yonge St Aurora ON L4G 1M2. The candidate will be expected to work on-site a minimum of 2 days per week and will be required to travel extensively within York Region & South Simcoe for client meetings staff training and meetings.
Program Profiles:
Rapid Access Psychiatry (RAP) is an evidence-based model of service for individuals ages 16 with mild-to-moderate depression and anxiety. The program provides individuals with an opportunity to attend a single appointment psychiatric consultation followed by a report with recommendations for treatment.
The Clinical Therapy Program is a group-based program that helps people with serious mental health issues to be more successful in day-to-day living by utilizing different forms of psychotherapy to help clients explore and manage unhelpful thoughts overwhelming/difficult emotions and problematic behaviours with the support of our team of Clinical Therapists. Our program helps clients develop and practice coping skills that help them gain control over unhelpful thinking patterns overwhelming/difficult emotions and unhelpful behaviours.
Job Profiles:
The Case Manager works with clients 16 years of age and older who are experiencing significant difficulties in day-to-day and role functioning as a result of serious and persistent mental illness and/or addictions. The Case Manager collaborates with the client to identify strengths and needs and develop long-term goals short-term objectives to meet these goals and concrete action steps to achieve the objectives. As part of this process individuals are assisted to acquire and use skills and to connect with formal and informal supports peer support professional services and resources to promote their recovery improve their quality of life and reduce the likelihood of relapse. Other duties as required.
Requirements:
- Degree with four years experience or Diploma with six years experience in a community based mental health organization; experience in homelessness sector is preferred.
- Current registration with OCSWSSW or a regulatory body under the Regulated Health Professions Act. Cannot be in the qualifying stages.
- A second language (French Cantonese Farsi Italian Korean Mandarin Punjabi Russian Tamil Urdu) reflecting the local community is an asset.
- Knowledge of supports and services in York Region and South Simcoe including formal and informal resources.
- A valid Canadian Full G drivers license a minimum $third party liability insurance and proof that insurance covers use of personal vehicle to transport clients.
- A satisfactory and current Vulnerable Sector Screening (Police Check).
Direct Client Service:
- Maintain a client caseload including extensive travel in rural areas
- Perform comprehensive assessments and develop individualized case management plans
- Establish collaborative client-directed relationships using empowerment and recovery principles
- Support goal development symptom management life skills and community integration
- Provide referrals advocacy counseling and crisis planning
- Support discharge planning and transition to appropriate services
- Support clients in wellness planning and appointments (diet exercise self-care)
- Respond appropriately to trauma and relapse using strength-based interventions
Group Facilitation:
- Co-facilitate groups and promote client participation in community activities
- Provide education on mental health recovery and available resources
Documentation:
- Maintain timely accurate and comprehensive client documentation in CRMS and Outlook
- Complete intake assessments case notes progress summaries and discharge plans
Community Engagement & Liaison:
- Collaborate with hospitals schools justice system and community agencies
- Educate the public agencies and institutions on mental health and referrals
Client Safety:
- Follow program guidelines and client safety frameworks
- Collaborate with clients on prevention and safety planning
- Support response to health and safety concerns in accordance with policy
