We are looking for a dedicated Care Manager to join Big Sur Health Center a leading healthcare provider in Big Sur. As a Care Manager you will play a crucial role in coordinating and overseeing the care of patients ensuring they receive high-quality and timely healthcare services. You will be involved in a variety of projects aimed at improving patient outcomes and satisfaction making a significant impact on the overall well-being of our community.
This role is vital to our organization as it directly contributes to the efficient delivery of healthcare services patient advocacy and care coordination. The Care Manager at Big Sur Health Center will work collaboratively with healthcare professionals to develop and implement care plans monitor patient progress and provide support to individuals in need. Responsibilities:
- Assess patients needs and develop individualized care plans.

Requirements:
- Bachelors degree in Nursing Social Work Psychology or related field.
- Previous experience in care management or healthcare coordination.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of healthcare regulations and best practices.
- Excellent organizational and time management abilities.
