The Opportunity
IFYS is seeking a passionate and collaborative Project Officer to support and strengthen housing networks across the Sunshine Coast region. This role focuses on person-centred care coordination for individuals with complex needs with the ultimate goal of reducing homelessness and sustaining tenancies.
Reporting to the Operations Manager Housing and Accommodation you will work both regionally and state-wide to build sustainable networks support care coordination groups and contribute to a community of practice that drives collective impact.

Key Responsibilities
- Support the Regional Care Coordination Group in addressing complex homelessness
- Work collaboratively across the housing sector to engage community and support in the Care Coordination Group building sustainable networks and structures
- Engage with Q Shelter and statewide facilitators to share regional insights and support collective impact
- Organise care conferences for individual households in response to referrals received from sector stakeholders
- Help identify and distribute tools that promote consistent high-quality care coordination
- Provide high-quality data to support evaluation improvement and impact measurement
- Accurate and timely recording of data in client management system
- Prepare and submit statutory reports in line with compliance requirements
- Deliver clear effective written and verbal reports to various audiences
- This role does not involve direct client case management
Skills and requirements
- Extensive experience and in-depth knowledge of the local homelessness sector including its services challenges and collaborative frameworks
- Exceptional relationship-building skills with stakeholders
- Strong administrative and organisational abilities
- Highly developed written and verbal communication skills
- A high level of computer and systems literacy
- Tertiary qualification within community services related field
Must hold or be willing to obtain if successful
- Valid Queensland drivers license with no restrictions
- QLD Working with Children Check (Paid Blue Card)
- Licensed Care Suitability Check (LCS2)
- National Crime Check (IFYS will complete a new check through our systems)
- Ability to provide two recent line manager references including your current employer
