Job Description

The Opportunity

Were inviting experienced Support Coordinator join our team in a contractor role. Youll retain your independence while gaining access to the support structure and systems of a values-driven registered provider; well give you the tools support and space to thrive.

This is a work-from-home role with travel required for client meetings generally within 30 minutes of your location.

About the Role

Youll support a caseload of NDIS participants with Level 2 funding helping them to:

  • Understand and use their NDIS plans
  • Access services and coordinate supports
  • Build skills for independence and self-advocacy
  • Navigate crises and reduce barriers to participation
  • Track progress and prepare reports for plan reviews
  • Strengthen formal and informal support networks

Youll be part of a team that genuinely values autonomy collaboration inclusion and excellence and that works hard to support you while you support others.

What Youll Bring

  • Relevant tertiary qualification (e.g. Social Work OT Psychology Community Services)
  • Minimum 2 years experience in Support Coordination or case management
  • Strong knowledge of the NDIS and disability sector
  • Excellent communication planning and problem-solving skills
  • Confidence using digital tools in a remote environment
  • A values-driven participant-centred approach

  • NDIS Worker Screening Check
  • Working with Children Check (VIC)
  • Professional Indemnity and Public Liability Insurance
  • Drivers licence and comprehensively insured vehicle
  • 100 points of ID and copies of relevant qualifications
