About the company

Working closely within the leadership team to support the aged community within a set geographic region to ensure care team members are delivering care services in line with industry standards & clients daily goals.

Due to the nature of this role travel will be required across the Western Brisbane region.

The role

As a Clinical Care Coordinator you will work closely with your care manager to ensure care team members are providing high quality care to clients within a set geographic across community services.

Having an engaging & collaborative communication style you will easily build relationships & links within the community with your clients staff & wider networks ensuring a personalised service is delivered to each client.

Holding an excellent understanding of industry standards & clinical indicators you will ensure that all staff are educated & supported in delivering care needs with the ability to highlight any areas of improvements.

Summary of Duties

  • Provide clinical guidance to staff surrounding client care
  • Undertake & review client assessments
  • Ensure accurate completion of relevant documentation
  • Motivation training & development of team members
  • Ensure compliance & quality indicators are met
  • Ensure funding models are met & updated

Skills and Experience

  • AHPRA registration within nursing or relevant discipline
  • Previous experience within a leadership role
  • Experience supporting clients within the community
  • Understanding & working knowledge industry standards
  • Ability to identify & implement areas of improvement
  • Excellent people leadership skills
