Job Description

Overview:

The Medical Concierge isto serve as the Otterbein representative for Perennial Advantage of Ohio for resident member(s) and resident family(s). Provides navigational assistance for Perennial Advantage members/family needs in order to optimize any and all care delivery. Address any questions or concerns from member/family specifically on care/service needs.

This position will be required to travel up to 50%; lodging will be provided if over 1 hour commute from home.

Responsibilities:

  • Review nurse notes (all facilities) for Perennial Advantage of Ohio (PAO) members each morning and report any issues to facility PAO Nurse Practitioners (NP).
  • Visit Perennial Advantage members weekly as able.
    • Document visit in the Electronic Medication Administration Record (EMAR). Update key facility staff and NP of any issues noted during visit.
  • Update PAO Utilization Management (UM) and Interdisciplinary Care Team with any transitions of care ie: hospitalization death and disenrollment.
  • Document in EMAR any hospitalizations/returns from hospital.
    • Review care plan. Ensure NP is aware of admissions/discharges.
  • Review orders for any upcoming appointments and submit referral requests for out-of-network physicians.
  • Gather and submit clinical notes for any upcoming procedures that require prior authorization.
  • Monitor for any potential skill in place (SIP) episodes.
    • Guide facility/NP thru process to ensure all clinical information reaches PAO UM department and monitors Portal for authorization approvals.
  • Update facility and NP of membership changes each month.
  • Complete Health Risk Assessments (HRAs) annually and with any new enrollment.
  • Assist NPs with obtaining information needed to complete Medication Therapy Management plans for members.
  • Maintain records from HRA/care plans authorizations etc.
  • Create and update care plans.
  • Attend care conferences even if NP can/cannot.
  • Assist with obtaining Perennial Advantage Permission to Contacts (PTC) as needed.
  • Attend all plan of care meetings (either in person or on phone) for Perennial members in nursing homes if NP unable. Document in EMAR and review care plan each quarter.
  • Coordinate plan of care meetings twice/year for the assisted living facilities. Ensure that facility staff NP family and member are aware of meeting. Document in EMAR and review care plan each quarter.
  • Attend weekly/scheduled calls each Friday and provide input on any SIP or Part A skilling episodes.
  • Attend and participate in morning meeting or clinical wrap up if in facility at time of meeting.

Qualifications:

  • Licensure/Certification: Current OH RN or LPN license.
  • Experience:
    • Minimum 5 years of experience in long-term care//nursing home environment required.
    • Experience in post-acute environments and/or case management significant plus
    • Experience with Electronic Medical Records; PCC experience highly preferred
    • The ability to work in multiple EMR systems.
    • Knowledge of MS Outlook and Excel.
