NEEDED: PHARMACIST
LOCATION: 100 U.S. HWY 40 & 4th Street Greenville IL 62246
SCHEDULE: Monday - Friday between 7:30 AM - 4:00 PM excluding federal holidays. Not to exceed 40 hours per week.
QUALIFICATIONS:To be considered for this position candidates must provide a copy of the following documents:
- US CITIZEN
- Shall possess the necessary skills abilities and credentials to perform professional Pharmacist services.
- Shall have current certification that permits him/her to practice and provide Pharmacist services in the State of Illinois
- Valid and current License that allows for the practice of Pharmacist.
- Proof of CPR certification.
- Evidence acceptable to the Clinical Director of recent testing for or immunity to Tuberculosis or other communicable diseases.
RESPONSIBILITIES: Shall provide Pharmacist services which will include but are not limited to:
- Serves as a resource person to the medical providers in the field of medicine as it relates to proper pharmacy practice patient profiles and review of prescriptions written
- Conveys proper pharmacy procedures and practices to medical staff via written memorandums direct personal contact and through personal observation
- Monitors drug therapy for contraindications interactions allergies and appropriateness
- Maintains patient medication records to include medication profile formats
- Responsible for the documentation of work performed
- Provides a balanced and efficient pharmacy service establishes and maintains standards of service and maintains maximum efficiency in economy of operations
- Makes independent decisions in the performance of duties and as such makes recommendations for revision or improvement as appropriate
- Responsible for the selection dispensing and preservation of drugs medicines and other therapeutic agents consulting updated references journals reports etc.
- Required to identify and ascertain differences between drugs as it relates to the therapeutic efficacy of the
- Product
- Periodically inspects drugs for adequacy overstocking arrangement storage proper rotation and deterioration
- Assists in maintaining an inventory of commonly utilized drugs and assumes responsibility for security of drugs in his/her custody
- Uses the proper techniques for the storage of drugs such as refrigeration or protection from heat and light
- Obtains familiarity of the Bureau of Prisons National Drug Formulary
- Must be alert in order to avoid any possible changes which may result in deterioration of the product and loss of its potency formation of toxic products or other undesirable effects in addition to adverse patient care
- Provides a wide range of drug information to medical staff
- Maintains adverse drug reaction (ADR) reporting system by investigating events to determine if an ADR has occurred
- Must be available to the medical staff relative to consultations of the patient treatment aspects of medication as it relates to drug compatibility mode of action drug reactions latest developments in the field of pharmacy and review of medication orders prescribed by the medical staff this is accomplished through individual consultations with the medical staff and review of medical records as indicated.
