You will be updated with latest job alerts via emailCommunity Health Center of the North Country seeks a compassionate and resourceful Case Manager to join our Federally Qualified Health Centers integrated care this role youll work directly with patients to identify needs reduce barriers and connect them with essential medical social housing and community resources.

From helping patients navigate insurance and billing to arranging transportation and translation services to facilitating access to vital community programs youll be making a meaningful difference every day.

Reports to: Clinical Development Manager
Rate of pay: $19.75 - $23.07 per hour
Work schedule: Full-time 37.5 hours per week. Primarily Monday - Friday 8:00 a.m. - 4:00 p.m. with some flexibility required based upon the needs of the Health Center and our patients. No nights no weekends no major holidays.
Position Summary:
The Case Manager will have knowledge of community services and resources to facilitate referrals for services for patients who present with needs or barriers to receiving care. This will include establishing eligibility for and gaining access to Federal State and local programs that provide or financially support the provision of medical social housing educational or other related services. The Case Manager works closely with a diverse population assisting with their needs while remaining culturally competent.
