Under the direct supervision of the Ryan White Part B Medical Case Management Supervisor the Specialized Medical Case Manager will provide strengths-based re-engagement individual level intervention that is focused on re-engaging HIV positive patients who are at risk of falling out of care or have fallen out of care.

Job Description

Job Duties:

  • 35%- Provides a range of services that link patients with medical care psychosocial services benefits and other supportive services through:

    • Assist the Outreach Coordinator with contacting patients via telephone and researching for updated contact information.

    • Works with patients as they are located and help prepare them to re-engage in medical care and clinic services.

    • Conducts Return to Care Assessment (RTC) on patients re-engaging in care and identifies their barriers to care.

    • Customizes services through the development of an Enhanced Care Plan (ECP) with the patient to re-align the patients perception/prior experience of service usefulness and accessibility barriers allowing the patient to set service priority

    • Communicate with management on necessary service enhancements

    • Emphasizes family/partner/other support systems

    • Provides goal-oriented time-specific Specialized Medical Case Management activities and support to encourage the patients participation in his/her own care plan.

    • Works proactively with patients until they are ready for less intensive traditional case management (i.e. SMCM must often take the initiative and follow the clients medical appointment and other service referrals very carefully)

    • Identifies emerging barriers that may prevent each patient from participating in care and treatment

    • Coordinates and links patients to services required to implement the ECP

    • Monitors to assess the efficacy of the ECP to include home/field visits as necessary

    • Reviews and re-evaluates the ECP frequently at a bi-weekly minimum

  • 25%- Maintains knowledge of community resources and methods to access services. Provides referrals and coordinates with community agencies to alleviate social determinants of health that prevent patients from engaging in care. Provides benefits counseling support and information including risk reduction and service literacy expansion. Assist patients in applying to state and federal programs such as presumptive disability Social Security Disability Medicaid and health insurance. Refer patients to housing services and other area resources. Coordinate and communicate with medical providers. Help with treatment adherence in support of medical directives.

  • 25%- Provide cultural and literacy-sensitive communication to clients with varied language and literacy barriers. Follow up and support patients toward individual competence and life skills to promote treatment adherence and remain in medical care. Maintain accurate documentation of services interactions contact information and other patient demographics in Provide Enterprise database and EPIC

  • 10%- Attend necessary trainings through the SC Department of Public Health within first year of hire and maintain thereafter. Attend monthly meetings through the SC Department of Public Health including monthly Outreach Meetings.

  • 5%- Other Duties as Assigned

Minimum Requirements: A bachelors degree and two years professional experience in human services or social services programs.

(Frequent) Ability to push/pull patients up to 300 wheelchairs etc. unassisted. (Frequent) Ability to lift and carry patients up to 300 lbs. assisted. (Frequent) Ability to maintain 20/40 vision corrected. (Continuous) Ability to see and recognize objects close at hand. (Continuous) Ability to see and recognize objects at a distance. (Continuous) Ability to match or discriminate between colors. (Continuous) Ability to determine distance/relationship between objects; depth perception. (Continuous) Good peripheral vision capabilities. (Continuous) Ability to maintain hearing acuity with correction. (Continuous) Ability to hear and understand whispered conversations at a distance of 3 feet. Ability to perform gross motor functions with frequent fine motor movements. (Continuous) Ability to work in dusty areas. (Infrequent) Ability to be qualified physically (by medical personnel) for respirator use initially and annually.

Additional New Requirements: Ability to deal effectively with stressful situations. Ability to learn and use new processes tools and equipment as required. Computer literacy. Ability to work overtime as required. Ability to work rotating shifts.
