Job Overview:
Rainbow Pride Youth Alliance (RPYA) is seeking a dedicated and experienced Care Coordinator to join our organization. The Care Coordinator provides our gender-diverse patients with assistance in navigating the health care system. The Care Coordinator will work within a team setting with other Rainbow Pride Youth Alliance staff to facilitate the prompt and efficient delivery of services. The ideal candidate for this role will have experience working with LGBTQ youth an understanding of the unique challenges that LGBTQ youth face especially youth struggling with homelessness and SUD and a passion for creating safe spaces for young people to explore their identities and build community. The program is rooted in community building culturally responsive trauma-informed care and harm reduction models.
Responsibilities May Include:
- The Care Coordinator will serve on the Clinical Team as a case manager thereby attending the requisite clinical meetings including supervision as well as programmatic meetings and All-Staff meetings.
- Assessment
- Provide initial screening intake assessment and coordination of care for youth during mobile/street outreach and at drop in Center
- Conduct comprehensive assessments to understand clients mental health needs including evaluating their emotional behavioral and social functioning and understand each persons health care needs and goals.
- Linkage of Care
- Ensure patients are established with both a primary care provider and mental health provider
- Coordinate referrals especially for MAT services as necessary
- Identify appropriate providers and coordinating referrals for other supportive services
- Coordinate insurance and benefits coverage
- Advocacy within the system of care in all aspects of care
- Provide linkages to community based resources
- Develop personalized treatment plans in collaboration with clients incorporating their goals strengths and areas of growth.
- Crisis Intervention and Support:
- Offer crisis intervention services to LGBTQ youth in times of immediate emotional distress or urgent mental health needs.
- Collaboration and Referrals:
- Collaborate with other professionals within RPYA such as psychiatrists case managers etc. to ensure comprehensive care for clients.
- Liaise with external mental health providers and community resources to facilitate referrals and access to additional support services as needed.
- Documentation and Compliance:
- Maintain accurate and confidential client records documenting assessments treatment plans and other relevant information.
- Adhere to ethical guidelines legal requirements and HIPAA regulations in all aspects of clinical practice.
- Prepare necessary reports and documentation for funding sources and regulatory agencies.
- Professional Development and Awareness:
- Stay updated on current research best practices and trends in LGBTQ mental health to enhance therapeutic interventions and knowledge.
- Engage in ongoing professional development activities including attending trainings workshops and conferences related to LGBTQ issues and mental health.
- Wellness Center Staff:
- Provide support in acting as a lead staff member in the Wellness Center including being responsible for opening and closing the Center as needed and overseeing drop in hours programs and events in the Center in conjunction with schedules created by the Wellness Center Director.
- Ensure that the Wellness Center remains clean organized and presentable to participants to ensure a functional and orderly space.
- As needed provide support for the front desk operations of the Wellness Center and ensure that program participants and community members arriving at the Wellness Center feel welcome and can access services.
- May facilitate groups and workshops for LGBTQ youth ages 11-26 virtually and in person on a variety of topics that include a range of activities and discussions that are engaging and relevant to youth participants.
Qualifications and Required Knowledge Skills Abilities:
- Demonstrated experience in supporting clients through the informed consent process for medical aspects of gender transition including access to hormones and surgeries.
- A clear commitment to actively work for social and racial justice.
- Experience collaborating with social service organizations to meet the needs of youth.
- Experience in youth development and rapport building.
- Bachelors degree in social work psychology public health or a related field (Masters degree or Nursing degree preferred).
- Experience working with gender-diverse youth and/or LGBTQ communities.
- Knowledge of healthcare systems and resources particularly those related to gender-affirming care.
- Strong communication and interpersonal skills.
- Ability to work effectively as part of a multidisciplinary team.
- Excellent organizational and time management abilities.
- Demonstrated commitment to social justice and advocacy for marginalized communities.
Compensation: $28 an hr/PT

Unity Hope is the community service branch of Divine Truth Unity Fellowship Inc an Inland Empire faith-based organization sponsored by the Unity Fellowship Church Movement. Rainbow Pride Youth Alliance (RPYA) is another project of DTUF which works closely with Unity Hope. RPYA is a grassroots mental health non-profit organization that was founded in 2001. It is an Inland Empire activity and support organization for Lesbian Gay Bisexual Transgender Queer Intersex and questioning youth (ages 13-20 years old) their families and their friends.
