We are looking for a compassionate and skilled Direct Support Professional who will take an active role in providing hands-on support coordinating health services and assisting nursing staff all to promote independence dignity and choice.
The Health Services Coordinator roleprovides individualized support to people with developmental disabilities in home community and health-related settings. This role includes developing and implementing active treatment programs supervising and assisting with training staff assisting with medical needs andproviding office coordination and support for RN and LPN staff within the Health Services department.
Support for People with Disabilities
Advocate for the health safety and quality of life of each person supported.
Assist with personal care daily living skills and community participation.
Accompany individuals to appointments and ensure follow-up care and documentation.
Implement individualized support plans (ISPs) and risk management plans.
Support people in making choices increasing independence and pursuing their interests.
Health & Wellness Services
Administer medications and treatments as directed.
Monitor medications and treatments ensuring accurate documentation.
Maintain and organize medical records for compliance and quick retrieval.
Support special diets nutrition and exercise programs as directed.
Respond appropriately to medical emergencies.
Provide education to staff on health-related topics including medication administration CPR First Aid Bloodborne Pathogens and Universal Precautions.
Office Coordination & Support for Nursing Staff
Schedule and coordinate medical dental therapy and other health-related appointments for people supported.
Serve as a point of contact for appointment confirmations follow-ups and documentation.
Maintain inventory of medical supplies health-related equipment and office supplies for the Health Services department ensuring timely ordering and replenishment.
Support RN and LPN staff by preparing documentation organizing files and ensuring information is readily available.
Assist nursing staff with scheduling communication and tracking of care needs.
Coordinate training sessions meetings and record keepingfor health-related topics.
Staff Supervision & Training
Provide leadership training and evaluation to staff in both group and one-on-one settings.
Assist in developing goals objectives and behavioral supports.
Teach best practices for behavior intervention that are respectful and minimally intrusive.
Ensure staff understand and follow health safety and emergency procedures.
Prefer prior experience supporting people with developmental disabilities or related conditions.
Experience supervising teaching/training evaluating and supporting staff.
Strong organizational skills for scheduling supply management and office coordination.
Ability to work independently and as part of a team.
Proficient in Microsoft Word Excel and Outlook.
Valid drivers license and acceptable driving record.
Prefer current CPR First Aid and Positive Behavior Support (PBS) certification or ability to obtain.
Ability to meet the physical requirements of the position with or without reasonable accommodation.
Willingness to provide hands-on care including lifting and supporting individuals as needed.
